The City of Crossville is requesting proposals for Standing Seam Roof Replacement at the Crossville Police Department located at 115 Henry Street in Crossville, TN. Scope of Services The proposed project consists on a Standing Seam Replacement at the Crossville Police Department including removing existing roof, installing new underlayment, and new standing seam roof with all accessories including a new gutter system. See attached Minimum Requirements. Services to be included are as follows: o Proposal to include a schedule for the project based upon the scope of services. o Proposal to include a lump sum price based upon the scope of services and allowances used in the proposal. Each proposal should include the following information for evaluation 1. Specialized experience or technical expertise of the firm in connection with the type of services to be provided. 2. Past record of performance on contracts with the community and other clients including quality of work, timeliness, and cost control. 3. Familiarity with local conditions. 4. List of at least (5) similar projects in TN in the last ten years. Provide Information concerning location, and project details. 5. Project schedule to be included in the proposal. 6. Lump Sum Price based upon the scope of services, and allowances used in the proposal. 7. Information concerning the manufacture, product information including brochures and any technical information relevant to the product proposed, and warranty information. 8. Include all relevant forms that are attached in this RFP including the RFP, Product Information Sheet, Certification of Contractor Regarding Iran Divestment Act, Certification of Contractor Regarding Equal Employment Opportunity (Title VI of the Civil Rights act), Drug-Free Workplace Affidavit, and Statement of Compliance Certificate - Illegal Immigrants. Each Proposal will be evaluated based upon the above information We reserve the right to accept or reject any or all proposals. Proposer understands that the Owner reserves the right to reject any or all proposals and to waive any informalities in the Request for Proposals. The proposer agrees that this proposal shall be good and may not be withdrawn for a period of 60 days after the scheduled closing time for receiving proposals.
