QUESTIONS concerning this Request for Quotation may be directed in writing to the Public Works Director CONTACT: James Creel, Public Works Director PHONE NO.: 573-657-2568 E-MAIL: jcreel@ashlandmo.us. The vendor hereby declares understanding, agreement and certification of compliance to provide the items and/or services, at the prices quoted, in accordance with all requirements and specifications contained herein. The vendor further agrees that the language of this RFQ shall govern in the event of a conflict with his/her proposal. The vendor further agrees that upon receipt of an authorized purchase order from the Purchasing Division or when a Contract is signed and issued by an authorized official of the City of Ashland, a binding contract shall exist between the vendor and the City of Ashland. The initial term of this Agreement shall be one (1) year, beginning on the Effective Date of April 1, 2024. Thereafter, this Agreement shall be renewable for up to two (2) successive one (1) year terms upon agreement of both parties. In no event shall this Agreement be binding on either party beyond three (3) years of the Effective Date. Actual work performed under this agreement shall be completed between the approximate dates of April 1 through October 31 of each year. Insurance Requirements: The Contractor shall not commence work under this Contract until they have obtained all insurance required under this paragraph and such insurance has been approved by the City, nor shall the Contractor allow any subcontractor to commence work on their contract until all similar insurance required of subcontractor has been so obtained and approved. All policies shall be in amounts, form, and with companies satisfactory to the City which must carry an A-6 or better rating as listed in the A.M. Best or equivalent rating guide.
