The Office of State Procurement has suspended in-person attendance by vendors at public bid openings for bids published by our office. This solicitation is for the Removal and installation of automatic door openers and card readers for the Louisiana Department of Health (LDH) located at Central Louisiana Supports and Services Center, 5400 Coliseum Boulevard, Alexandria, LA 71303. The job is to be performed in accordance with the enclosed scope of work and specifications. Please see attachments for information about insurance, builder's risk, bond, specifications, jobsite visit, and contractor's license requirements. Please print all attachments to ensure all documents related to this solicitation are reviewed prior to bidding. All documents associated with this solicitation should be included in bidder's submission. The contractor shall perform fully, entirely, and in satisfactory manner the work contracted, within the number of calendar days stipulated in the proposal and the contract. Time will be assessed against the contractor beginning with the date of the notice to proceed. Contractor shall provide all labor and materials (except for existing card readers that are to be moved) in order to install four automatic door openers, move four card readers, and install one card reader and lock set for the Louisiana Department of Health (LDH). Contractor shall be responsible for cleanup of any debris. Contractor shall install one modular Strike for Mortise and cylindrical lockset, stainless steel, satin, field selectable voltage 12 or 24vdc and 12 volt to 24VAC. -Contractor shall run compatible wire from Transitional Family Life Center building #20 to Transitional gym building #21 in order for nurse's station card reader to work. -Contractor shall perform a system test to make sure all equipment is functioning properly.
