Direct all questions regarding this Solicitation, and any questions or Issues relating to the accessibility of this Solicitation (Including the appendices and exhibits to this Document, and any other document related to this Solicitation), to: Kolman, John. The purpose for this Request for Quotes (RFQ) is to procure one (1) Single ADA Water Cooler with Bottle Filling Station and three (3) replacement filtration filters for the State of Hawaii, Department Health facility located at: Kona Health Center 75-1015 Haukapila Street Kealakekua, HI 96750 The VENDOR shall cover all steps: A. Review Exhibit 'A', Specifications for the Elkay Enhanced EZH20 Bottle Filling Station as reference to the warranted product or similar. B. Complete and Submit Offer Form attached to the RFQ. The price quote shall be the all-inclusive cost to the STATE, consisting of all related costs and applicable taxes for providing the specified product. C. Submit Company Quote with product specifications. D. Attach a Copy of their Certificate of Vendor Compliance and W-9.
