Remodeling of a medical facility in Mammoth Lakes, California. Completed plans call for the remodel of a medical facility.
At the Board meeting currently scheduled for June 20, 2024 Southern Mono Healthcare District dba Mammoth Hospital (the "District" or "SMHD") is requesting bids for remodeling the medical imaging department of the hospital building located at 85 Sierra Park Road, Mammoth Lakes, CA 93546 (the "Project"), all in accordance with the Bid Documents. General Description of Project: Remodeling the current medical imaging department of the hospital building located at 85 Sierra Park Road, Mammoth Lakes, CA 93546 for modernization and workflow optimization. Scope includes renovations to the existing front office and provider work areas of the medical imaging department, which will include but not be limited to demolition, framing, drywall, roofing, stucco, plumbing, electrical, mechanical, structural, and other specialties as required. This project will also involve prep work and facilitation of the installation of a new CAT Scan modality in the remodeled space. Project will require remodel work to be done while maintaining the existing clinical functions of the department as directed by Owner. Bidders are advised that the Authority Having Jurisdiction for this project is the California Department of Healthcare Access and Information (HCAI). Bidders are notified that this is a public project and public contracting and prevailing wage rules apply. The contract will be awarded to the lowest responsible bidder, who shall give security as required hereby. The successful lowest responsible bidder shall furnish to the District a bid security bond in an amount not less than ten percent (10%) of the bid in favor of the District. As a condition precedent to contract award the successful bidder shall be required to furnish a payment bond and a faithful performance bond on the forms provided in the Instructions to Bidders. Bidders shall be licensed contractors in the State of California and shall be skilled and regularly engaged in the general class or type of work called for under the Contract. Each bidder shall have a Class B California Contractor's license. Each Bidder shall be required to comply with all local, state, and federal laws and regulations governing the Project, and the bidder shall demonstrate sufficient experience with like projects. The bidder shall be required to provide a non-collusion affidavit. No Bidder may withdraw its bid for a period of 60 days after the date set for opening of bids. The Bidder with whom the District enters into a contract shall agree therein to indemnify, defend, and hold the District financially free and harmless from any and all liability which may be occasioned as a result of the activities of the contractor, including without limitation, personal injury, property damage, completed operations, and contract liability. In order to effectively implement the indemnification and hold harmless provisions, a certificate of insurance must be returned with the bid. Said policy or policies shall include the District as a named insured, and such policy or policies shall be primary to any insurance carried by the District, with the insurer being admitted in the State of California with an A.M.
