CLARIFICATION DEADLINE 5/15/2024 2:00 PM CONTACT INFORMATION mmccauley@waterburyct.org The Work of Lead Hazard Reduction and Healthy Homes Intervention. Two separate bids required. Bids must be enclosed in an opaque sealed envelope and plainly marked with the name of the Project Title The Lead Hazard Reduction and Healthy Homes Intervention 49 Chapman Avenue and shall contain the name and address of the Bidder on the envelope. Each Bid shall be submitted in accordance with the Instructions to Bidders. All bids in excess of $50,000.00 be accompanied by a Bid Security in the amount of ten (10) percent of the Total Bid Price. The Bidder to whom a contract is offered, must furnish to the City, if that contract has a total cost greater than $50,000, a 100 percent Performance Bond with a surety company acceptable to the City and in a form acceptable to the City. In the City's sole discretion, it may also require a 100 percent Payment Bond and/or other additional security with a surety acceptable to the City and in a form acceptable to the City. Complete instructions for filing Bids are included in the Instructions to Bidders. After review of the factors set forth in the Instructions to Bidders, the City reserves the right to reject any and all Bids, to make an award, or to decline to make an award. The United States Department of Housing and Urban Development (HUD), Office of Healthy Homes and Lead Hazard Control has awarded the Waterbury Department of Public Health and the Healthy Homes Program funding to carry out activities associated with a Lead Based Paint Hazard Control Program The Waterbury Healthy Homes Program (WHHP) is undertaking a lead-based paint abatement project located at 49 Chapman Avenue in Waterbury, Connecticut. The Site building consists of a two (2)-family residence. The interior ofthe dwelling units along with the common areas and exterior areas have been comprehensively tested for lead-based paint. Toxic levels of lead-based paint were identified on various components and surfaces. There are no known lead-based paint abatement orders on the inspected building No child under the age of six (6) years old resided at the Site at the time of the inspection. Full abatement has been specified for the entire Site building at the request of the Waterbury Healthy Homes Program. All work specified in the Scope of Work must be performed by a State of Connecticut Department of Public Health (CT DPH) licensed Lead Abatement Contractor. completion-August 21, 2024 (25% of the work with DAS certified Small and Minority owned businesses and 25% of that work with DAS certified Minority, Women and/or Disabled owned businesses.) The contractor must demonstrate good faith effort to meet the 25% set-aside goals.
