CR Number: 2110353 The Project involves the interior renovation and associated infrastructure upgrades of an existing NYS DOH Article 28 licensed delivery site. The Project will substantially renovate 7,000 s.f. within the existing 15,000 s.f. first floor public/clinical areas. The interior renovation will include a new (6) exam room, Urgent Care suite, (36) seat waiting room, (10) intake workstations, staff support areas with plumbing, mechanical, and electrical system upgrades. New ceilings, walls, flooring, ACT ceilings, furniture, workstations, and medical and I.T. equipment will be provided throughout. The building façade renovations will consist of new Main Street storefront entrance doors, masonry restoration, and exterior lighting. The center, located at 6 Henry Street, will be wholly occupied by Sun River. The center is being designed to have the following functionality: i. 1st floor: Medical exam rooms, soiled utility, clean utility, secure medical storage, provider/nursing station, consult rooms, adult and pediatric waiting areas, intake/reception area, and associated staff and patient toilets. ii. 2nd floor (No work in this area): Administrative offices, and associated staff toilets and support spaces. b. The Sun River Health Satellite will provide the following services with Sun River employed staff: i. Comprehensive Primary Medical Care for children, adults and seniors ii. Women's Health (including Prenatal and Family Planning Services) iii. Comprehensive Dental Care for children, adults and seniors iv. Integrated Behavioral Health (LCSW counseling for medical patients who are not SMI) v. Phlebotomy vi. Only topical anesthesia, such as lidocaine on skin for certain Infections, and Novocain for dental procedures will be administered at the site. vii. No general radiology will be located on site. Only digital radiography for dental services will be provided. viii. Sterilization of non-disposable instruments will be conducted in accordance with state standards and Sun River's protocols within the rooms designated on the schematics.
