The State Laboratories Division is requesting a quote to replace 2 boiler blow down tanks. Direct all questions regarding this Solicitation, and any questions or Issues relating to the accessibility of this Solicitation (Including the appendices and exhibits to this Document, and any other document related to this Solicitation), to: Schanzenbach, Stephen. The Department of Health, State Laboratories Division (SLD) is requesting a price quotation to replace two (2) boiler blowdown tanks at 2725 Waimano Home Road, Pearl City, HI 96782. Please provide a price quotation for the following: Scope of Work: 1) Remove and dispose of existing boiler blowdown tanks and connected piping up to 10 feet from the blowdown tank. 2) Purchase and deliver two (2) new boiler blowdown tanks to SLD. Contractor is responsible for determining the proper sizing for the new blowdown tanks. Please see attachments for information regarding the connected boilers. 3) Mount new blowdown tanks and connect to system. Contractor is responsible for providing all materials, equipment, and labor required. 4) Equipment shall be warranted for a minimum of 1 year. Contractor shall leave work area clean and is responsible for removing materials and other debris generated because of the work. Contractor shall follow all federal, state, and local laws and requirements including but not limited to Chapter 104 (Prevailing wage law) Contractor shall possess a valid Hawaii State boiler contractor's license (C-4).
