Bridge #10 - Clay County, Indiana Located on Kennedys Crossing Rd over Otter Creek The Board of Commissioners reserve the right to reject any and all bids or to waive any irregularities in bidding which, in its judgement, will be in the best interest of Clay County, Indiana. All bidders are required to submit their proposal on the printed forms contained in this packet and include additional documentation as requested below. No bidder may withdraw their bid for a period of twenty-one (21) days after the date of the bid opening. All bids are to include: 1. A bid bond or cashier's check equal to ten percent (10%) of the bid 2. Bid specifications 3. A completed "Contractors Bid for Public Work - Form 96" which includes a Non-Collusion Affidavit 4. A Certificate of Insurance 5. A Declaration of Coverage Page 6. A completed Affidavit under IC 22-5-1.7 E-Verify Program, if applicable Successful bidders will also be required to furnish a Performance Bond for the amount of the contract.
