PROJECT DESCRIPTION/SERVICES REQUESTED The Professional Services Contractor (PSC) is to provide a proposal for Phase 400 Preliminary Design, Phase 500 Final Design, Phase 600 Construction Administration Office Services and Phase 700 Construction Administration Field Services for the MDOC WHV - Roof Replacements Project. The buildings MDOC would like to complete roof replacements for include: 1. West Housing Unit 1 2. Prisoner Services Building 3. Field House 4. MSI 5. East Kent Hall The PSC is asked to include the below reimbursable(s) as part of their proposal: Asbestos Testing & Roof Cores: $10,000.00 Available drawings and asbestos reports will be provided to the selected PSC. The PSC shall verify all existing conditions. Please NOTE: o Proposal responses MUST be uploaded to SIGMA VSS. Please enter the total cost for all phases as the bid amount. o Firms should only submit one (1) attachment (being less than 25 MB) for proposal submission. The attachment is to be the technical and cost proposal combined. o Do not wait until just before the 2:00 p.m. solicitation deadline to submit your proposal response.
