Renovation of a mixed-use development in Fort Harrison, Montana. Completed plans call for the renovation of a medical facility.
The purpose of amendment 0003 is to officially and effectively make the following changes/updates listed below: 1. Post clarification on the question about a substitute item. Attachments 1-5 of the items not approved. 2. All proposals are due on 10/11/2024 to the Contract Specialist, Jesus A. Villegas at jesus.villegas@va.gov, by 10:00AM MST. All other terms and conditions remain the same. The purpose of amendment 0002 is to officially and effectively make the following changes/updates listed below: Post the site visit meeting sheet, the Surgical Corridor Specifications, the Surgical Corridor Flooring update, the Surgical Corridor ICRA, Surgical Corridor PCRA and the Epoxy Flooring previously used information. Post the answers to the RFI's. All proposals are due on 10/11/2024 to the Contract Specialist, Jesus A. Villegas at jesus.villegas@va.gov, by 10:00AM MST. All other terms and conditions remain the same. The purpose of amendment 0001 is to extend the proposal deadline and add documents, The Montana VA Health Care System requires a project to replace the flooring in the surgical suite located on the second floor. The intent of this project is to remove existing vinyl sheet flooring (approx. 4,000 sqft) and install new VA approved high build glazed epoxy coating. The current floor finishes in these areas are deteriorated and should be replaced with new infection control compliant finishes. The sheet vinyl floor will be removed in its entirety, the concrete subsurface prepared as required to accept a new poured resinous flooring system with integrated resinous wall base. Contractor shall provide all necessary labor, equipment, materials, and supervision to complete all required project work in accordance with the provided SOW, Specifications and Drawings. The project is located at the Montana VA Health Care System at 3687 Veterans Drive Fort Harrison, MT 59636 on the second flooring of the Surgical Suites. Project Name: Surgical Suite Floor Renovation Project: 436-24-121 This is a competitive requirement set aside 100% for Service-Disabled Veteran Owned Small Business (SDVOSB) Project magnitude: between $500,000 and $1,000,000 The NAICS code is 236220 - Small Business size standard is $45M. A non-mandatory pre-bid site visit will be held on August 29,2024, at 2:00PM MST. All interested parties please meet Mr. Tyler Padilla at room 208 in Building 141. Cut off for submission of RFIs/Questions in relation to this solicitation must be submitted in writing via e-mail to jesus.villegasva.gov and received no later than September 05,2024, at 10:00 AM MST. All bidders must submit a bid guarantee as part of their bid package. Additionally, the Awardee must be able to obtain the necessary level of Performance and Payment Bonds. Instructions, Conditions, and Notices to Offerors. All submitted proposals must be received in email by the closing date and time in block 13a below. Please include the solicitation number 36C25924R0122_1 in the subject line. Proposals shall be submitted via email in accordance with Part I/Section B, to be considered for this project. Proposals received in any other format will result in the offeror's proposal not receiving further consideration. In accordance with FAR 52.204-7, System for Award Management (SAM), Prospective contractors shall be registered in the SAM database prior to close of solicitation. In accordance with P.L. 116-283, this acquisition is 100% set aside for Service-Disabled Veteran Owned Small Business concerns. See VAAR 852.219-73, VA Notice of Total Set-Asidefor Verified Service-Disabled Veteran-Owned Small Business for information in regard to requirements in order to be considered. Offerors must be both VISIBLE and VERIFIED by the Small Business Administration (SBA) prior to proposal receipt. Failure to be both VERIFIED, and VISIBLE by the SBA prior to contract award will result in the offerors proposal being deemed non-compliant. All offerors are urged to contact the SBA and submit the aforementioned required documents to obtain SBA (VetCert) verification of their SDVOSB status if they have not already done so. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
