Clay County is requesting quotes from qualified contractors to provide housing rehabilitation services at the following location: 401 Harrison Street Green Cove Springs, FL 32043 Question Submission Deadline:July 22, 2024, 4:30pm Question submitted Electronically Substantial Completion shall be obtained within 60 days from Notice to Proceed. Final Completion shall be obtained within 105 days from Notice to Proceed. The Contractor shall be assessed Liquidated Damages (LD) of $100.00 for each day past final completion date set by the contract. Scope- Below is the list of recommended repairs. A. HVAC 1. Tune up and deep clean coils and condenser. B. Carpentry 1. Repair 5 X 9 guest bathroom floor (toilet flange/ leak). The vanity shall be removed and retained. Cut off flange for replacement by plumber. Repair joists as needed. Block all edges including bathtub margin. Use properly sized lengths of screws from under side of tub edge to avoid damaging tub. Replace flooring 23/32 sanded plywood to prep for sheet vinyl. 2. Install sheet vinyl on new bathroom floor and prime baseboards. 3. Install R-19 Batt Floor insulation and vapor barrier under repaired bathroom floor. 4. Termite treatment on effected area(s). 5. Repair and/or replace termite damages at garage back door. C. Electrical 1. Upgrade smoke alarms to current code in all three (3) bedrooms and install a combo smoke/CO within ten (10) feet outside of bedroom doors. A minimum of communicating/lined ten (10) year batteries or new hard wired to be installed. 2. Add a GFCI outlet protection in laundry rooms, garage, and exterior outlets. D. Paint 1. Patch guest path drywall and re-paint wall and trim. Plumbing1. Remove guest bath standard toilet and replace with ADA comfort height elongated bowl toilet when repairs are complete. 2. Remove and replace two (2) sinks and one (1) toilet supply valves and supply hoses, Remove and replace sink drains, popup and faucet. 3. Repair allowance for replacement of toilet drain/flange. 4. Remove and replace Master bath sink, drains/popup, supply lines and faucet. F. General Requirements 1. The contractor is responsible for submitting this owner prepared work write up to the City of Green Cove Springs Building Department and applying for, paying, and receiving a building permit prior to starting any work.
