This Contractor's Special Statement of Experience must be provided on the Contractor's letterhead. If your firm provides inaccurate or incomplete information your firm will be found non-responsive. If your firm receives negative project references they may be found non-responsive. To be eligible for consideration of award, the Contractor's Special Statement of Experience must include: 1. Documentation that the bidder has successfully completed the following: a. A minimum of three (3) and a maximum of five (5) comparable traffic striping projects, with a construction value of $80,000 or greater, completed in the past five (5) years that demonstrate the bidder's ability to successfully complete the project with respect to project size, cost, use, complexity, etc. DO NOT incl projects currently under construction. b. All submitted projects must be within the immediate nine Bay Area counties. 2. Include the following for each project: (a) project name, (b) project address, (c) owner's name, (d) contract value at award, (e) total Change Order value, (f) completion date, (g) Owner's Representative email address and current phone number and (h) describe how project is comparable to project being bid. 3. All traffic striping projects on all UC Campuses and National Labs (LBNL, LLNL) within the last 5 years. LICENSE REQUIREMENTS: The successful Bidder will be required to have the following California current and active contractor's license at the time of submission of the Bid: very effort will be made to ensure that all persons have equal access to contracts and other business opportunities with the University within the limits imposed by law or University policy. Each Bidder may be required to show evidence of its equal employment opportunity policy. The successful Bidder and its subcontractors will be required to follow the nondiscrimination requirements set forth in the Bidding Documents and to pay prevailing wage at the location of the work. This project is subject to compliance monitoring and enforcement by the Department of Industrial Relations.
