Site work for a mixed-use development in Eustis, Florida. Completed plans call for site work for a playground / park / athletic field; and water / sewer project.

These Contract Documents to include but not limited to: Installation of 70-foot length, 8-inch diameter pipe piles filling the centers of the installed pin piles with concrete and encasing the installed pin piles in concrete, setting up and removing all formwork required, providing pile-driving equipment and methodology and perform all required testing and monitoring as indicated on the construction drawings and project technical specifications. It is anticipated that the bulk of the work will need to be performed from the waterward side of the existing bulkhead wall structure, utilizing a barge. Demolition and site preparation of the site that may include hauling on disposal of any debris from any demolition activities to prepare the site while taking all necessary precautions to avoid damage to bulkhead wall and other nearby structures. Please note that the Ferran Park seawall structure and the nearby bandshell structure are listed on the US National Register of Historic Places. Furnish the services of a State of Florida licensed land surveyor for the field layout of all work. Provide necessary manpower, equipment and materials needed to maintain federal, state, and local regulatory compliance during all phases of construction. The St. Johns River Water Management District (SJRWMD) has determined that this project is exempt from permitting requirements; a copy of the verification of exemption letter is attached. The City and the Engineer are currently working on renewing the US Army Corps of Engineers permit. Provide necessary manpower, equipment and materials needed to retain the services of third party seismic monitoring firm to measure and evaluate vibration through the earth created by vibratory and/or impact hammering of steel pin piles required to complete the project; assess potential adverse impacts to the existing bulkhead and other nearby structures. Please note that the Ferran Park seawall structure and the nearby bandshell structure are of historical value. Provide necessary labor, equipment and materials needed to Repair of the two (2) floating docks in Ferran Park, which are outside of the Ferran Park Seawall area. BID EVALUATION PERIOD - Sixty (60) Days After Bid Opening Substantial Completion = 120 Days After Notice To Proceed Final Completion = 150 Days After Notice To Proceed BID WITHDARAWAL - Bids may be withdrawn or modified up to the advertised date and time of opening. Withdrawal request shall be in writing. The City reserves the right to require proper identification of any party making such a request. No bid may be withdrawn subsequent to bid opening ACCEPTABLE BID FORMAT - Bid format provided by the City is the only acceptable format on which a vendor may return his bid. Bids submitted on any other format shall be disqualified. Any additional information relative to the bid should be submitted on separate format. EXECUTION OF BID - Each bid sheet shall contain a manual signature of an authorized representative in the space (s) provided. All information shall be legible and either written in ink or typed. All corrections (additions or erasures) made by the bidder to any entry shall be dated and initialed in ink. BID ADDENDA - All changes in connection with this bid will be issued by the City in the form of a written addendum. Signed acknowledgement of receipt of each addendum shall be submitted with the bid response. CONDITIONAL BIDS - The City of Eustis reserves the right to reject bids containing any additional terms and conditions not specifically requested in the bid solicitation. ACCURACY OF BID - Bids are read aloud and made a part of the public record of the City. Therefore, it is necessary that information presented is accurate and that by which the awarded bidder will complete the contract. Should there be a discrepancy between the unit price and an extended total, the unit price shall prevail. BONDS - Bid Bond - 5% Performance Bond - 100% Payment Bond - 100% 3. Bid Security - Bids must be accompanied by a certified or cashier's check or bid bond made payable to the City of Eustis in an amount equal to five percent (5%) of the total bid amount. Bid deposits of the unsuccessful bidders will be returned upon award of the bid by the Eustis City Commission. The bid deposit of the successful bidder will be retained by the City of Eustis until an agreement is executed. Should the successful bidder fail to enter into an agreement with the City of Eustis within ten (10) working days, the bid security will be forfeited to the City as liquidated damages.
