Contractors shall comply in full with all the safety regulations and requirements of OSHA, MIOSHA, State Authorities, Local Authorities, School policy, and the Construction/Project Managers requirements. Failure to do so will result in the contractor being removed from the site. 2. Smoking and use of Drugs or Alcohol on School property is Strictly Prohibited! 3. Drinking water is the responsibility of the contractors for their crews. 4. Contractors are required to perform daily and weekly cleanup including proper dust control generated by their construction activities. 5. Dumpsters supplied by the owners. Concrete or masonry products are not permitted and are the responsibility of the contractor to dispose. 6. Contractors are to contact Miss Dig three (3) days prior to any excavation. 7. All contractors are to cooperate with inspection and testing agencies hired by the owners. Additional costs associated with any failed inspections or tests will be passed on to the contractor whose work did not pass. 8. This project will require permits and inspections. If not existing, contractors are responsible for pulling, paying, and scheduling inspections as required by the governing authorities. 9. All contractors are responsible for layout and field dimensions associated with their scope of work. 10. Contractors are responsible to remove mud and debris from streets and walkways as a result of their work activities. 11. Contractors are responsible to review all drawings, specifications and include all work as defined in their respective bid proposal. 12. Contractors should include sales tax and bond premiums within their bids. 13. Contractors are responsible to protect existing finishes and newly installed products from damage. 14. Contractors are responsible for all lifting, hoisting, scaffolding, etc. required to complete their work and schedule all deliveries during normal business hours. 15. Contractor supplied materials shall be stored in contractor provided storage units unless arrangements are made with the CM/PM in advance. 16. All contractors are to ensure the project is safe and secure at the end of each work day. All windows, doors and gates should be locked at the end of all work days. 17. Contractors are responsible for snow and ice removal required to complete their scope of work. 18. All openings made by contactors shall be covered and maintained by the contractor until permanent finishes are completed. 19. Firestopping of openings in walls shall be done by the contractor who created the opening. 20. The owner(s) and CM/PM shall obtain and pay for the building permit only. 21. This project does not require payment of prevailing wages. 22. Vehicular traffic routes shall be maintained and open to the greatest extent possible to minimize disruption of school vehicular traffic and busing. 23. Contractor shall to the greatest extent possible, include trade students in the planning and installation of their scope of work. 24. Project hours are from 7 A.M. to 7 P.M. on weekdays. Project hours on weekends, holidays, etc., shall be by permission of the owners or their representatives. Notify the Construction/project manager at least five (5) days prior to bid of ambiguities, inconsistencies, or errors discovered upon examination of the proposal/bidding documents, site or local conditions. Submit requests for clarification or interpretation of the bidding documents in writing. Interpretation, correction, or change of bidding documents will be done by addendum, all other forms will be non-binding 2. Residential Septic System No Pre-Proposal/bid conference will be held. Rempert Road Tawas City, MI 48763 Email: jflory@ioscoresa.net Cell Phone: 989.820.7708 PROPOSAL DIVISION: #1 SEPTIC SYSTEM The scope of work includes but is not limited to, the development and installation of the Septic System for a new seventeen hundred square foot (1700 sf) residential home including three bedrooms and two bathrooms. List of requirements for successful completion of the project by the contractor, but is not limited to: a. Contractor shall pay for all permits, fees, and inspections required by governing jurisdictions. This includes all state and local permits to ensure code compliance for the project and work being completed. b. Provide a complete septic disposal system that includes waste lines, tank, field, and materials required for installation as follows but is not limited to, the project specifications: c. Bidders should include all materials and labor to completely install the Septic system that will meet or exceed the State and Local code requirements and successfully pass all inspections. d. Bidders should include a New two compartment 1500 gallon tank and accessories. e. Materials required to complete a 1500 sf field (30x50) f. Bidders shall include all piping and sewage lines that meet or exceed existing code requirements. g. Bidders are responsible for all excavation, aggregate, stone, sand, fill, and seeding required for installation and finish of the system. h. Bidders shall review district health department plans and specifications and coordinate installation with Don Roussou. i. Contractor shall coordinate delivery and storage of materials with the CM/PM prior to delivery. j. Contractor is responsible for the removal and disposal of all materials and ensures they are disposed of properly. k. Contractor is responsible for returning the landscape to the condition prior to installation of the septic system. SECTION - 5 PROPOSAL DIVISION: #2 DEEP WELL The scope of work includes but is not limited to, the Installation of the homes Deep Well system for a new seventeen hundred square foot (1700 sf) residential home. List of requirements and specifications for the bidder/contractor for successful completion of the project as follows, but is not limited to: a. Contractor shall pay for all permits, fees, and inspections required by governing jurisdictions. This includes all state and local permits to ensure code compliance for the project and work being completed. b. Contractors should include all materials and labor to completely install the Deep Well system that will meet or exceed the State code requirements and successfully pass all inspections. c. The contractor should include in their bid to drill approximately 230 feet below grade for bidding purposes. d. Bidders will work with owners to obtain and test samples of water for EAGLE. e. Bidders should include 5 inch PVC casing, shale trap, pitless adaptor, Myers .5 Hp Pump, pressure tank, one inch drop pipe, one inch 200 psi waterline, cable 12/2 wire with ground, and other materials to complete the installation. f. Contractor shall coordinate delivery and storage of materials with the CM/PM prior to delivery. g. Contractor is responsible for the removal and disposal of all packaging materials and ensures they are disposed of properly. h. Bidders shall coordinate with the district health department No. 2 ensuring well installation meets or exceeds their inspection.
