See attached sheet. o Legal formation of Bidder (i.e., sole proprietor, partnership, corporation). o Date Bidder's company was formed. o Legal authority to conduct business in Massachusetts (i.e., business license number). o Insurance, Workman's Compensation policy, Liability insurance policy. o General Contractor or Home Improvement License no. o Certificate of Foreign Corportion. o Comply with prevailing wage rates for all workers on site. See attached sheet. o L.E.M. (labor, equipment, materials) cost for each phase. o List/itemize all exclusions o Schedule of proposed payments
