1. Scope of Work: Annual check to ensure emergency generators are running to manufacturers specification to meet Joint Commission Standards. See attached list of inspections, checks, work to be done. Equipment: Caterpillar 3516C Standby Diesel Generator - Model CAT 3516C-HD Kohler Generator - Model 2000REOZMD Performance o The contractor shall provide all materials, parts, and labor being requested. o The contractor shall provide all inspections and testing for Cooling System Overhaul to ensure in working order after cooling system flush and the replacement of all cooling system components and coolant. o The contractor to provide certified technicians to handle on work for each listed "equipment" listed in "Scope of Service". o Contractor to fill out "Generator Inspection checklist" form for both Generator(s), and submit to Contract Manager. General Compliance General guideline to follow while on HSH Property o HSH shall have the right to terminate the contract at any time by giving the Contractor a thirty (30) day written notice. o All work shall be done between 7:30am - 3:00pm, Monday through Friday, except State Holidays, unless agreed upon with Contract Holder. o The duration of this work shall be no longer than ninety (90) workdays, excluding rain out days unless agreed to by Hawaii State Hospital Facility Plant Manager. o Contractor to provide safety signs, barricades, and other devices necessary for the safety and convenience of the general public. Contractor shall at all times conduct his/her work to assure the least possible disruption to normal public traffic (vehicular and pedestrian) and allowing for the free flow of traffic and public right of way. o Shirts are required to be worn at all times on the work site, smoking is not allowed anywhere on the property, no radios or headsets are allowed. o Adverse conditions which may require major field changes not stated in the scope of work for this project must be reported to the Hawaii State Hospital Facility Plant Manager prior to commencement of continuation of work. o Contractor shall re-execute any work that fails to conform to the requirements of this agreement and shall immediately remedy and defects due to faulty workmanship by the Contractor. Should the Contractor fail to comply to perform the services and to deduct such costs from monies due to the Contractor. 2. Vendor Compliance o Proof of compliance obtained through Hawaii Compliance Express (HCE). The vendor's name on the Certificate of Vendor Compliance (CVC) must exactly match the vendor's name under which the quote for this solicitation is submitted. A copy of CVC must be submitted with quote for verification. The vendor is responsible for maintaining compliance. Non-compliance may result in a vendor not receiving an award, delay of payment, or cancellation of award. o The interested offeror must be a registered company to do business in the State of Hawaii. 3. Compensation and Payment Schedule The Contractor shall submit complete invoice upon completion of project to Hawaii State Hospital for the services provided in accordance with Section 1, "Scope of Services." The Contractor shall include all applicable taxes, shipping, fees of any kind when submitting the via electronic bid. 4. Performance Period: The Contractor shall provide the product required under the contract from October 18, 2024 through October 17, 2025, unless the contract is sooner terminated. When in the interest of the State, HSH may terminate the contract at any time and for convenience by providing eight (8) weeks prior written notice to the Contractor. 5. Qualification o Valid State of Hawaii Electrical C license o 5 years' experience with emergency generator maintenance 6. Submission and Award All submittals shall be made via HIePRO. The following documents must also be completed and submitted by the vendor. o Offer Form OF-1 o Valid State of Hawaii Electrical C license o Liability Insurance o Qualification Form Selection and award shall be contingent on the lowest and most responsible and responsive offer. HSH reserves the right to cancel this procurement or award at any time and for convenience by providing eight (8) weeks prior written notice to the Contractor.
