Contract term: 60 Days of Award Location: 117 Perry Hill Road Shushan NY 12873 NYS Labor Law requires that all contracted employees preforming maintenance, repairs or capital improvements in state owned buildings receive the prevailing wage and supplements rate for the occupation in which they are working. If this is a public work contract covered by Article 8 of the Labor Law or building service contract covered by Article 9 thereof, neither Contractor's employees nor employees of its subcontractors may be required or permitted to work more than the number of hours or days stated in said statutes, except as otherwise provided in the Labor Law as set forth in prevailing wage and supplement schedules issued by the State Labor Department. Certified payroll record MUST be submitted at the time of invoicing. The purpose of asking questions during the site visit is to obtain clarification regarding the enclosed Project Scope. In the event that modifications are needed to the Project Scope, the modifications will be distributed by the CAPITAL DISTRICT DDSO Business Office to all contractors as an ADDENDA. Contractors are to prepare and submit their quotes based on the written Project Scope and any modifications made in writing through the ADDENDA. The Capital District DDSO is seeking bids/quotes for the above referenced project (see attached specifications). SCOPE OF WORK includes, but is not necessarily limited to the following: Site Visit 1. Site visit and property survey shall be conducted. 2. Survey the property, noting locations of wells, property lines, affected bodies of water, and elevations. All pertinent information shall be included in the design drawing. 3. Conduct at least two percolation tests, as per NYS DOH Test Guidelines, in the proposed location of the absorption field. DESCRIPTION: This project shall provide OPWDD with P.E. stamped design drawings of a residential septic system. The site shall be evaluated for, but not limited to, percolation rate and proper setbacks. The design shall comply with NYS DOH Residential Wastewater Treatment Design, as well as current NYS Uniform Code. The design shall be submitted to the NYS DOH for review. Work is to be completed within 60 days of award. Pricing must adhere to guidelines set forth in NYS OGS Award Number: PGB-23063 GR, RM. Refer to attached specification for the following scope of work. The CAPITAL DISIRICT DDSO intends to award the contract to the most responsible and responsive vendor that provides the best value and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude. The CAPITAL DISTRICT DDSOO reserves the right to reject any and all quotes or award based on MWBE SDVOB certification. The awarded vendor will be required to submit certificates of Liability, Automotive, Worker's Compensation and Disability Insurance as well as a completed Lobbying Law Form. Please be prepared to submit these in a timely fashion. The CAPITAL DISTRICT DDSOO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition.
