Project no.CD25147 225.-2-22.1 OPWDD's Capital District DDSO intends to procure the installation of a septic system (per the attached scope of work) pursuant to its discretionary purchasing authority. The system shall follow the provided stamped drawing for the septic system and shall comply with NYS DOH Residential Wastewater Treatment Standards, as well as current NYS Uniform Code. Work is to be completed within 60 days of award. Pricing must adhere to guidelines set forth in NYS OGS Award Number: PGB-23063 GR, RM. Refer to attached specification for the following scope of work. Full prints shall be provided to the awarded bidder. SCOPE OF WORK includes, but is not necessarily limited to the following: Installation 1. Excavation of site for installation. 2. Destruction of existing in place tank per prints 3. Installation of trenching, required piping, vaults with associated internals, septic tank and stone/pipe leach fields according to included prints. 4. All installations shall conform to local standards and the provided drawings. 5. All fees for NYS DOH will be the responsibility of the vendor. 6. Project will be considered complete upon approval of installation by Warren County DOH and OPWDD Plant Superintendent Site visits by owner shall be expected during construction for compliance of scope of work.
