Contract - Sewer Service for the McAfee Center in Fort Monmouth BID FOR: "Contract 216 - Sewer Service for the McAfee Center in Fort Monmouth" The envelope shall plainly set forth the name and address of Bidder. The project consists of: o Maintenance and protection of traffic. o Installation of approximately 40 LF of 6", and 210 LF of 8" PVC sanitary gravity piping and associated appurtenances. o Installation of new sanitary sewer cleanouts, manholes and connection to existing sanitary sewer were shown on the plans. Bids shall be enclosed in a sealed envelope, addressed to the Executive Director of the Two Rivers Water Reclamation Authority, and shall be clearly marked on the outside lower left-hand corner, "Contract 216 - Sewer Service for the McAfee Center in Fort Monmouth." Bids shall bear the name and address of the Bidder on the outside. Bids shall be accompanied by a certified check, cashier's check or bid bond payable to the Two Rivers Water Reclamation Authority as a guarantee that if the Contract or Agreement is awarded to the Bidder, that the Bidder will enter into an Agreement. The amount of the guarantee shall be ten percent (10%) of the base bid but shall not exceed $20,000.00. Agreement of Surety, List of Owners, Bidder's Affidavit, Non-Collusion Affidavit, Bidders Statement of Qualifications, Public Works Contractor Registration Act Certificate, Subcontractor Listing, Equipment Certification, Acknowledgement of Addenda, State of New Jersey Business Registration Certificate, Affirmative Action, Disclosure of Investment Activities in Iran, Americans with Disabilities Act of 1990 Acknowledgement, Certificate of Equal Opportunity, American Products Certification and Prevailing Wage Acknowledgment documents are also mandatory submittals. Bids shall be made on the standard Proposal Form in the manner designated therein and required by the specifications. Bidders are required to comply with the requirements of P. L. l975, C. 127, (N.J.S.A 10:5-31 et seq. and N.J.A.C.17: 27) Affirmative Action and P. L. 1963, C. 150, Prevailing Wage. The Two Rivers Water Reclamation Authority reserves the right to waive any informality in or to reject any and all bids. Bids may not be withdrawn within sixty (60) days after the date of said opening. *The list of planholders and/or interested bidders is not being tracked by the stakeholders involved with this project.
