RFQ - Westfield High School Science Lab Floor REQUEST FOR QUOTATION 1. This quotation is being solicited under the provisions of M.G.L. Chapter 149 2. Please quote to the Westfield Public Schools, on this form, for all supplies/services listed on this request for quotation. 3. All quotes must be received either by facsimile, email, or by mail. All quotes must be clearly marked RFQ#2025-S003 Westfield High School Science Lab Floor. Vendors are responsible for ensuring that their quote has been received. The Westfield Public Schools is not responsible for quotes not properly received. Scope of Work Please see the below specifications for the work to be completed: 1. Grinding to start when school is not in session due to noise restrictions 2. Remove existing paint 3. Mechanically prepare concrete substrate including all edgework by use of dust free, vacuum assisted diamond-grinding equipment. Steel enclosed shot blasting as needed. 4. Vacuum floor 5. Install 4" integral epoxy cove base, 104 LF 6. Fill minor divots and cracks using repair system 7. Sweep, detail, vacuum 8. Apply MVB Primer pigmented 9. Install epoxy coating broadcast with decorative flake. (Basalt 1/4) 10. Apply epoxy grout coat clear 11. Apply urethane protective topcoat clear 12. Allow floor to cure for minimum 24-72 hours at an average temperature of 68 degrees F (or higher). 13. The work must be completed as soon as possible upon award and prior to December 31, 2024. 14. Massachusetts prevailing wage rates must be paid.
