Buckhorn School Road Project All Contractors and Subcontractors Are Required to Be Registered With Sam.- Gov. Funding of This Project Will Be Provided by the City of Buckhorn Through Funds Made Available From Fema. The Work to Be Bid Consists of: Buckhorn School Road Project: (37.34814, -83.47410) Remove and Replace Asphalt Surface on Buckhorn School Road, Buckhorn, Kentucky, 63 Feet Long X 18 Feet Wide X 3 Inches Deep (10.5 Cy of Asphalt). The Scope Includes Mobilization, Demobilization, Maintain & Control Traffic, Roadway Excavation, and Replace Asphalt Surface. The Gps Coordinates of the Project Location is 37.34814, -83.47410. Contractor Shall Fur- Nish All Materials, Equipment, Labor, and Reclamation Necessary to Complete the Construction of the Water Facilities Described Above and Below. Non-compliance With This Requirement May Jeopardize Receipt of Federal Funding. Documentation of Borrow Sources Utilized is Required at Closeout. Time of Completion for the Contract is 45 Calendar Days. Other Requirements: Contractor is Required to Obtain Any Necessary Federal, State, and Local Environmental Permits Prior to the Start of Construction. Contractor is to Complete Repairs Utilizing Good Construction Practices. In Order to Repair a Damaged Road Surface, a Wider Section O the Road Must Be Repaired by Removing Past the Actual Damage to Ensure Its Properly Repaired and There Are No Weak Spots That Might Be Missed. Ky Dot Requires 50 Lf on Both Ends to Be Feathered Into the Damage Area. Striping and the State Edge Key Will Be Used Where Applicable. All Borrow or Fill Material Must Come From Pre-existing Stockpiles, Material Reclaimed From Maintained Roadside Ditches (Provided the Designed Width or Depth of the Ditch is Not Increased), or Commercially Pro- Cured Material From a Source Existing Prior to the Event. For Any Femafunded Project Re- Quiring the Use of a Non-commercial Source or a Commercial Source That Was Not Permitted to Operate Prior to the Event (E.g. A New Pit, Agricultural Fields, Road Rows, Etc.) in Whole or in Part, Regardless of Cost, the Contractor Must Notify the City of Buckhorn, Fema and the Recipient Prior to Extracting Material. Fema Must Review the Source for Compliance With All Applicable Federal Environmental Planning and Historic Preservation Laws and Executive Orders Prior to a Subrecipient or Their Contractor Commencing Borrow Extraction. Consultation and Regulatory Permitting May Be Required. Bid date notes. All Bid Proposals Which Have Been Submitted in Accordance With the Conditions Set Forth Will Be Publicly Opened and Read Aloud at the Perry County Courthouse, Hazard, Kentucky.
