The following property MAY require modification of its existing HSTS, or require extension of its existing HSTS, or may require replacements of its HSTS. It currently has an aeration, dosing, and leach field system. Contractor to determine remedy. 1. 130 Cynthia Ann Drive (07-0123.015) The County expects contractors to team with a soil scientist and licensed professional engineer to form a team that will ultimately test soils and propose the appropriate HSTS solution for each parcel. The Cost proposal shall include two separate guaranteed maximum prices for items A., and B. Preconstruction Services: 1. Soil evaluation and design of the appropriate HSTS solution for each property replacement. 2. Determination of modification or improvement, if necessary, for 130 Cynthia Ann Drive 3. Prepare and Submit Permit Application(s) to Scioto County Health Department Scope of Work: 1. Preconstruction Services: o Collaborate with the County to review and refine project plans. o Provide cost estimates, scheduling, and constructability reviews. o Assist in obtaining necessary permits and approvals. Proposal Requirements: 1. Company Information: o Firm's name, address, and contact information. o Overview of the firm's history and experience with similar projects. 2. Project Team: o Resumes of key personnel who will be assigned to the project. o Description of the team's relevant experience and qualifications. 3.
