Professional Services Requested: Phase II Environmental Site Assessment (ESA) for the Beaver location. The purpose of this RFSQ is to obtain statements of qualifications from companies. The intent is to award a contract under Administrative Rule R33-105 for the Small Purchases of Professional Service Providers and Consultants. The small purchase threshold for this project is $100,000.00. Do NOT include pricing when you submit your qualifications. The purpose of this Phase II ESA is to obtain sound, scientifically valid data on the presence, concentration, and/or absence of hazardous substances and vessels that may have released hazardous substances and to support property development. The parcel(s) that comprise the Subject Property for the Phase II ESA are listed below and presented in Figure 1. The Subject Property's primary address is understood to be 150 South Main Street. o Beaver County Parcel # 03-0005-0011, approximately 0.20 acres in size o Beaver County Parcel # 03-0005-0015, approximately 0.45 acres in size Note: The area of the Subject Property is approximately 0.65 acres.
