To submit a bid, all relevant documents and materials pertaining to this advertisement, including the bidder's name, address, all contact numbers, e-mail addresses, the items listed hereinabove, fully executed non-collusion affidavit, business relationships affidavit and the contract executed with lump sum price included, and should be placed in an envelope marked "Concrete Crushing Services") and delivered or mailed to City Clerk before the end of time for the receipt of proposals. The City will contract with the bidder who presents the lowest and best bid or may reject all proposals or take other action in its best interest.
