Rebid 1 - Hydraulic Dock Leveler Maintenance and Repair Services Description: Contractor shall provide maintenance and repair services, including installation and/or replacement of these units, and like kind, which includes ongoing preventive maintenance. In addition, Contractor shall provide all material, labor, tools, travel and per diem, non-consumable supplies, transportation, equipment and every other item of expense necessary to provide maintenance and routine (preventive) hydraulic dock leveler maintenance and repairs for four (4) hydraulic dock levelers for the California Department of Corrections and Rehabilitation (CDCR), Pleasant Valley State Prison (PVSP), 24863 West Jayne Avenue, Coalinga, CA 93210. DVBE participation of no less than three percent (3%) is mandatory for all bids exceeding $15,000. These requirements apply whether conducting business as a for-profit or nonprofit individual, partnership or corporation. In order to be deemed responsive and eligible for award of the Agreement, the bidder must attain the prescribed goals. Insurance Requirements Insurance as required herein shall be a condition of the State’s obligation to pay for services provided under this Agreement. Prior to approval of this Agreement and before performing any work, Contractor and any subcontractor shall furnish to the State evidence of valid coverage. The following shall be considered evidence of coverage: A certificate of insurance, a “true and certified” copy of the policy, or any other proof of coverage issued by Contractor’s insurance carrier. Binders are not acceptable as evidence of coverage. Providing evidence of coverage to the State conveys no rights or privileges to the State, nor does it insure any State employee or insure any premises owned, leased, used by or otherwise or under the control of the State. It does, however, serve to provide the State with proof that the Contractor and any subcontractor are insured at the minimum levels required by the State of California The purpose of these letters is to provide bidders the opportunity to ask questions and/or provide feedback to CDCR on the specifics of the IFB and/or DVBE requirements. While some input may be incorporated into the IFB, remarks and explanations submitted may not necessarily change provisions of the IFB. Any modifications to the IFB as a result of these inquiries will be documented by an addendum and forwarded to all bidders. The State reserves the right to terminate this Agreement subject to thirty (30) calendar days written notice to the Contractor. Contractor may submit a written request to terminate this Agreement only if the State should substantially fail to perform its responsibilities as provided herein.
