A unit price contract will be awarded for the construction of the 2025 Terre Haute CCMG Project illustrated on the associated plan sheets. The project consists of milling and repaving 13th Street from E Margaret Avenue to E Voorhees Street; milling and repaving 14th Street from 8th Avenue to Beech Street, milling and repaving 15th Street from Park Street to Crawford Street; milling and repaving 16th Street from Putnam Street to Crawford Street; milling and repaving 17th Street from Wabash Avenue to Liberty Avenue; milling and repaving 22nd Street from College Avenue to Wabash Avenue; milling and repaving 22nd Street from Locust Street to 8 th Avenue; milling and repaving 23rd Street from College Avenue to Poplar Street; milling and repaving 23rd Street from Locust Street to 8 th Avenue; milling and repaving Barton Avenue from Park St to Wabash Avenue; milling and repaving Beech Street from N 14th Street to N 21st Street; milling and repaving College Avenue from S US 41 to S 25th Street; milling and repaving Kent Avenue from Wabash Avenue to Liberty Avenue; milling and repaving Tippecanoe Street from N 21st Street to N 25th Street; removal of old railroad ties, milling, and resurfacing 17th Street from Hulman Street to Franklin Street; and reconstruction of Center Street from Washington Avenue to College Avenue. The plans depict the scope of work for the project. All work must comply with the City of Terre Haute Standards and INDOT Specifications where applicable, or as specified in technical specifications, and the requirements of the Americans with Disability Act where applicable. Bidders shall submit State Board of Accounts Form 96 (Questionnaire and Financial Statement). Bidders shall submit a list of similar projects completed. 2025 Terre Haute CCMG Project All bids must be submitted on the prescribed State Board of Accounts Bid Form No 96, which includes the completed execution of a non-collusion affidavit and proposal form included in specifications. Bidders must be INDOT Certified to submit. Bid Security: Any person, firm or corporation who submits proposals MUST file with their proposals a Bond or Certified Check in the amount of 5% of said bid made payable to the Board of Public Works and Safety. To ensure faithful completion of the contract entered into with the City of Terre Haute by the successful bidder, said bidder will be required to give a Performance Bond and Payment Bond due at the time of the execution of the contract, in the full amount of the contract price with sufficient surety thereon. The Board of Public Works and Safety reserves the right to reject any or all bids presented and waives technicalities as to procedures and to award a contract on the bid that, in its judgment, is the most advantageous to the City of Terre Haute, Indiana.
