Each bidder is required to furnish with its proposal, a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. Each Proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. The owner intends and requires that this project be completed in 45 calendar days from the notice to proceed. This project consists of milling and profiling to establish a desired cross-slope along with 2 of resurfacing of approximately 1,765 linear feet of Murray Avenue from Jackson Street to Robinson Road in the City of Campbell. Also included in the project is the replacement of approximately 4,775 square feet of sidewalk. The following items are included as part of the project: clearing and grubbing, catch basin adjustment/reconstruction, manhole adjustment/reconstruction, pavement repair, roadway signage replacement, curb ramp construction, and linear grading. All contractors and subcontractors involved with the project will, to the extent practicable use Ohio products, materials, services, and labor in the implementation of their project. Additionally, contractor compliance with the equal employment opportunity requirements of Ohio Administrative Code Chapter 123, the Governor s Executive Order of 1972, and the Governor s Executive Order 84-9 shall be required. Bidders must comply with the prevailing wage rates on Public Improvements in Mahoning County and the City of Campbell, Ohio as determined by the Ohio Department of Commerce, Bureau of Wage and Hour Administration, 614.644.2239. No bidder shall be considered a competent and responsible bidder or eligible to be awarded the contract to which this Notice applies, if the bidder is listed on the Auditor of State s database as having a Finding of Recovery as that term is defined in O.R.C. 9.24. Bids shall be submitted on forms provided and shall state the Unit Price or Lump Sum as indicated by the blanks provided; therefore and shall be enclosed in a sealed envelope marked on the outside Murray Avenue Improvements with the contractor s name, address, phone number, email address and contact name. The City of Campbell reserves the right to reject any or all bids and to waive any formalities or irregularities in bidding.
