Site work for a road / highway in Mustang, Oklahoma. Completed plans call for site work for a road / highway.
Faxes or emails will not be accepted. The equipment shall be new and unused, and shall meet or exceed the attached specifications. Bid price shall include delivery and installation of the specified equipment as well as all other materials and services as described in the Bid Specifications. Equipment shall be installed at multiple intersections in Mustang, OK. Bids will be reviewed by staff and recommendations made then placed on the next regularly scheduled meeting for Council's approval. Bids shall be made in accordance with the Notice to Bidders, Instructions to Bidders, Specifications and Bid Form, which are on file in the office of the City Clerk. All Bidders must sign Business Relationship, Anti-Collusion, and Certificate of Non-Discrimination Statements. Each of these statements shall be submitted with the sealed bid. Failing to submit signed statements with their Bid shall be cause for rejection of the Bid. The City of Mustang is exempt from payment of Federal, State and local taxes. The successful Bidder must also show proof of proper Liability Insurance in an amount of no less than $1,000,000.00 and Workman's Compensation in an amount and form that complies with Oklahoma law prior to execution of the contract. All insurance must be issued by a company licensed to provide such insurance within the State of Oklahoma. The City of Mustang reserves the right to reject any or all bids or to waive any informality in the bidding. The City of Mustang further reserves the right to award the bid to the lowest responsible bidder. The City of Mustang further reserves the right to issue change-orders on awarded projects in compliance with applicable law. For further information, contact David Russell, City of Mustang Project Manager, (405) 376- 4521 ext. 1219 between the hours of 8:00 AM and 4:30 PM, Monday through Thursday or 8:00 AM and 11:30 AM on Fridays./ The following items must be included in the bid, in the following order: 1. Bid Proposal Form 2. Statement of Qualifications 3. Anti-Collusion Affidavit 4. Certificate of Non-Discrimination 5. Business Relationship Affidavit The Bidder is responsible for reviewing this list of required documents and assuring that each and every listed document is properly completed, signed and submitted with the Bid. Failure to properly complete, sign, attest/notarize and submit all of the listed documents invalidates the Bid. The City of Mustang shall award the project to the lowest Bidder whose proposal is deemed to be in the best interest of the City. Bidders shall insure that all pricing in the bid is valid for a period of ninety (90) days from the time the bid is opened until notice to proceed is given by the City. The City shall give notice to proceed within a timely manner after award by the Mustang City Council. After the date of the notice to proceed, the contractor shall have ninety (90) calendar days to move on site and proceed with work. After moving on site, the contractor shall have fourteen (14) calendar days to complete the work. If work is not completed within one hundred four (104) calendar days, the contractor shall be fined $500 per day for every calendar day thereafter, until work is completed to the satisfactory of the City of Mustang. The City shall make payment on the project after the project is completed.
