DESCRIPTION OF WORK: In the two children's restrooms: replace flooring and wall covering up to 4'-0" AFF, provide one accessible lavatory and one accessible toilet for children 3-5 years old in each of the restroom, remove existing baby-changing table and existing wall-mounted radiant heater, building a partial-height furring wall adjacent to accessible toilets, and patch-and-repair. Anticipated NTP date is 04/01/25 and all work must be completed on or before 07/25/25. BIDDER QUALIFICATIONS: To allow the University to evaluate bidders, each bidder must submit with its bid, a Contractor's Special Statement of Experience. This Contractor's Special Statement of Experience must be provided on the Contractor's letterhead. If your firm provides inaccurate or incomplete information your firm will be found non-responsive. If your firm receives negative project references they may be found non-responsive. To be eligible for consideration of award, the Contractor's Special Statement of Experience must include: 1. Documentation that the bidder has successfully completed as prime contractor, the following: a. A minimum of three (3) and a maximum of five (5) comparable projects with a construction value at award of $75,000 or greater, completed in the past seven (7) years that demonstrate the bidder's ability to successfully complete the project with respect to project size, cost, use, complexity, etc. DO NOT include projects currently under construction. b. All submitted projects must be within the immediate nine Bay Area counties (Defined as Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Solano, and Sonoma counties, California). 2. Include the following for each project: (a) project name, (b) project address, (c) owner's name, (d) contract value at award, (e) total Change Order value, (f) completion date, (g) Owner's Representative email address and current phone number and (h) describe how project is comparable to project being bid. 3. All comparable projects on all UC Campuses and National Labs (LBNL, LLNL) within the last 5 years.
