High storm flows in winter 2023 led to the collapse of the culvert, crib wall, and failure of the slope above the outlet at stream level. Permittee will replace the existing culvert and headwall, restore the roadway embankment, and place rock rip-rap at the culvert inlet and outlet. Additionally, areas onsite will be replanted with native plants.
