Site work, paving and new construction of a civil project in Cutler Bay, Florida. Completed plans call for the construction of a bridge / tunnel; for site work for a bridge / tunnel; for paving for a bridge / tunnel; road / highway; road / highway; and water / sewer project.

Work under this Contract includes furnishing of all supervision, labor, materials, tools, equipment and performing all operations required to construct the Work in accordance with the Contract Documents. Work includes but is not limited to the following: o Replacement of existing bridge No. 874431 with a wider bridge section. o Widening of existing C1-N canal under new proposed bridge including dredging and widening of canal bottom. o Roadway approach improvements to include Signing & Pavement Markings, Milling & Resurfacing, roadway widening, new drainage features, guardrail installation, etc. o Will require close coordination with all utility stakeholders and Miami-Dade Water and Sewer Department throughout Construction. o Particular close coordination with Miami-Dade Water and Sewer Department will be required for proposed 8" water main along north side of proposed bridge. LOCATION OF WORK: A. The location of work to be performed under the terms of this Contract shall be as follows: 1. Bridge No. 874431 along Caribbean Boulevard over C-1-N Canal Crossing at SW 103 Avenue to SW 103 Place. B. The exact location and limits of construction are as shown on the Plans accompanying these Contract Documents. The Contractor shall furnish to Department of Transportation and Public Works, 111 NW 1 Street, Miami Florida 33128, Certificate(s) of Insurance which indicate that insurance coverage has been obtained which meets the requirements as outlined below: 1. Worker's Compensation Insurance as required by Florida Statute 440 2 Commercial General Liability Insurance for $1,000,000 each occurrence, $2,000,000 aggregate. Products/completed operations. Miami-Dade County must be included as additional Insured for any and all work. 3. Automobile Liability Insurance covering all owned, non-owned and hired vehicles for $1,000,000 combined single limit. 4. Umbrella providing excess coverage over underlying(s) B and C for a minimum $3,000,000 each occurrence/aggregate. Terms to be not more restrictive than underlying coverages. 5. Contractor's Pollution Liability $1,000,000 each occurrence $2,000,000 aggregate. Miami- Dade County must be included as additional insured. 6. Professional liability for $1,000,000 each occurrence $2,000,000 aggregate covering claims arising out of the rendering or failure to render professional services or provision of products. 7. Builders' risk insurance for the total value of the project on a replacement cost basis. All Risks or special perils including wind. Miami-Dade County must be included as a named insured or loss payee. Acceptable FDOT prequalification(s) necessary to perform the work specified in the Project's Scope of Work shall include the Minor, or Major Bridge Work Class. SBE/Goods & Service Subcontractor Goal is 2.0% in the commodity/service area of: o (NIGP 33055) Fencing, Temporary, safety barriers (For Construction and Other Industrial or Safety Uses) o (NIGP 90976) Site Work (Incl. Site Clean-Up) o (NIGP 79050) Sodding services With the passing of House Bill (HB) 705 the project will have no SBE-Con goal and no responsible wages. SBE-S Requirements YES Percentage: 2.00% Liquidated Damages: YES $$ Per Day: $2,667.00 For RPQ's less than $10,000, if no LD rate is specified, the County reserves the right to assess actual damages in lieu of LDs.
