The Contractor is required to submit only the Proposal, which includes: 1. Completed Bid Item Schedule 2. Certified check or acceptable proposal bond in an amount not less than 5% of the bid made payable to the City of Hiram, Georgia 3. List of references from the past 5 years a. Experience performing similar services for a municipality or county, within the past year, is preferred. b. Experience performing similar services for a municipality or county, with an accumulated experience of four years or longer, is preferred. Company History from the past 10 years or longer 4. 5. Proof of General Liability and Workman's Compensation Insurance meeting the limits that the City has in place for its own insurance. Bid Documents, other than sealed bids, are open to the public at the Hiram City Hall Administration Office, 217 Main Street, Hiram, Georgia. The Contractor shall not subcontract, transfer, assign, or otherwise dispose of the contract, or any portion thereof, without the written consent of the City. The successful Contractor shall secure and pay for necessary approvals, permits, assessments, and charges required for the conduct of the contract, as required by local, state, and federal regulations. Contractors must comply with Title VI of the Civil Rights Act of 1964, the Anti-Kickback Act, the Contract Work Hour Standard Act, and the National Occupational Safety and Health Act of 1970. Contractors must certify that they do not, and will not maintain, or provide for their employees any facilities which are segregated on a basis of race, color, creed, or national origin. The City of Hiram reserves the right to reject all bids and to waive formalities. Any claims for cost incurred by any Contractor in preparation of any part of or total package for this project will not be considered for reimbursement by the City of Hiram.
