Site work and paving for a bridge / tunnel in Columbus, Indiana. Completed plans call for site work for a bridge / tunnel; and for paving for a bridge / tunnel.

Bids deadline will not be considered but will be returned to the bidder unopened. Only proposals from those CONTRACTORS who are registered on the Indiana Department of Transportation's current listing of Pre-qualified Contractors for item U (A) "Bridge: Highway Over Water" will be considered. Any bids submitted by CONTRACTORS not approved for this item on the list will be returned to the bidder unopened. The work to be performed and the proposals to be submitted shall include a bid for all general construction, Labor, material, tools, equipment, taxes, (both federal and state), permits licenses, insurance, service costs, etc incidental to and required for this project. Each individual proposal must be enclosed in a scaled envelope bearing the title of the project and the name of the bidder firmly affixed. The bidder shall affix identifying tabs to the following sheets of each proposal as mentioned below: Form 96 Non-Collusion Affidavit Bid Bond Employment Eligibility Verification Certification Receipt of Addendum (If Applicable) Itemized Proposal Financial Statement Each individual proposal shall be accompanied by a certified check or acceptable bidder's bond, made payable to the Bartholomew County Treasurer, in a sum of not less than ten percent (I0%) of the total amount of the proposal, which check or bond will he held by the said Bartholomew County Treasurer as evidence that the bidder will, if awarded a contract, enter into the same with the OWNER. upon notification from him to do so within ten (10) days of said notification. Failure to execute the contract and to furnish performance bond to Bartholomew County, Indiana, will be cause for forfeiture of the amount of money represented by the certified check, or bidder's bond, as and for liquidated damages. Form 96, as prescribed by the Indiana State Board of Accounts, shall be properly completed, and submitted with bid proposals. Doc note: Copies of the Proposal, Specifications, Contract Documents and Plans must be obtained from Reprographic at online or contact David Henkle, P.E. at dkhenkle@dlz.com for further information. Cost for obtaining the Contract Document & Plans from Reprographic will be around $40.00 without tax. Payments and costs of Contract Documents are non-refundable. It shall be the responsibility of the individual to periodically check for addendums posted online. Contract Documents and Plans must be obtained through Reprographic to be eligible to bid on this contract. The Commissioners at their discretion reserve the right to waive any and all informalities in the bidding_ All bids submitted shall be good for 120 days from the opening of the bids.
