Repair and/or replacement of portions of concrete pavement on Buchholzer Boulevard. LOCATION OF WORK The areas for this project shall be within the right-of-way of Buchholzer Boulevard from Howe Avenue to the Sam's Club driveway. The Engineer shall determine the extent of work. SCOPE OF WORK The Contractor shall furnish tools, equipment, materials, labor and services for the complete removal and replacement of concrete streets and curbs and all construction necessary and associated therewith. The work shall include but not be limited to the following: 1) Removal of existing concrete pavement with integral curb and island. 2) Filling and compaction of sub-base material. 3) Installation of approved geotextile fabric, as directed. Tensar Triax TX-160 only. 4) Installation of four-inch (4") underdrains as directed. 5) Installation of 12-inch (12") ODOT 304 limestone aggregate base. 6) Installation of 8" concrete pavement with dowels and rebar as required 7) Installation of a concrete island 8) Rebuild existing catch basins 9) Traffic control during all operations including police officers and permanent striping. 10) Landscaping and restoration as required. Bidders must use the printed forms provided therefore, as none other will be accepted. Each proposal must contain the full name of the party or parties making the same, and all parties interested therein, and must be accompanied by a bond or certified check in the sum of five percent (5%) of the total amount of the bid, on a solvent bank, as a guarantee that if the bid is accepted, a contract will be entered into. The Director of Public Service reserves the right to reject any or all bids and to waive any informality in any proposal. Bids will be received only from parties that have obtained a recorded bid set of drawings and specifications. The successful bidder must post a Performance, Payment, Maintenance bond in the amount of one hundred percent (100%) of the total amount of the bid. The City of Cuyahoga Falls shall apply a Local Bid Preference to this invitation as outlined in Section 181.08 of the Codified Ordinances.
