Annual resurfacing and/or repair of various streets in the City of Cuyahoga Falls. The Contractor shall furnish tools, equipment, materials, labor and services for the complete grinding, patching and resurfacing of various streets and all construction necessary and associated therewith. The work shall include, but shall not be limited to the following: 1a) On streets to be planed and resurfaced: 1) Plane up to two-inches (2") to allow placement of two lifts of asphalt, unless directed. 2) Install all ADA handicap ramps including inserts as directed. 3) Full depth repairs as directed. 4) Replace curbs and drive approaches as directed. 2) Repair, replace and/or adjust manholes, valve boxes and monument boxes as directed by the Engineer. 3) Pave indicated streets with three quarters of an inch (3/4") scratch course as directed by the Engineer. 4) Pave indicated streets with one and one-quarter inches (1-1/4") of 448 Type I Fiber Reinforced Asphalt unless directed by the Engineer. 5) Traffic control during all operations including police officers and temporary striping are included in the price for maintenance of traffic. Bidders must use the printed forms provided therefore, as none other will be accepted. Each proposal must contain the full name of the party or parties making the same, and all parties interested therein, and must be accompanied by a bond or certified check in the sum of five percent (5%) of the total amount of the bid, on a solvent bank, as a guarantee that if the bid is accepted, a contract will be entered into. The Director of Public Service reserves the right to reject any or all bids and to waive any informality in any proposal. Bids will be received only from parties that have obtained a recorded bid set of drawings and specifications. The successful bidder must post a Performance, Payment, Maintenance bond in the amount of one hundred percent (100%) of the total amount of the bid. The City of Cuyahoga Falls shall apply a Local Bid Preference to this invitation as outlined in Section 181.08 of the Codified Ordinances.
