The City of Phoenix Water Services Department (WSD) is seeking a Contractor to provide Horizontal Lifeline (HLL) Inspections and Vertical Fall Protection System (VFPS) Inspections and potential repairs at various WSD facilities. Contractor(s) must provide all necessary labor, equipment, materials and transportation to conduct inspections and repair of various sized Horizontal Lifelines (HLL) and Vertical Fall Protection Systems (VFPS). The City of Phoenix Water Services Department (WSD) is seeking a Contractor to provide Horizontal Lifeline (HLL) Inspections and Vertical Fall Protection System (VFPS) Inspections and potential repairs at various WSD facilities. Contractor(s) must provide all necessary labor, equipment, materials and transportation to conduct inspections and repair of various sized Horizontal Lifelines (HLL) and Vertical Fall Protection Systems (VFPS). 3.2. Background The City of Phoenix requires preventative maintenance, and repairs to Horizontal Lifelines and Vertical Fall Protection Systems (VFPS) on an annual basis. The Horizontal Lifelines and Vertical Fall Protection Systems are used for a variety of reasons and are vital to daily operations within the Water Services Department operations. The equipment ensures the safety of the WSD employees and assists in performing their job functions. Inspections will be scheduled on an annual basis and repairs will be conducted as requested by the City. Inspection and repairs must comply with manufacturer's, Occupational Safety and Health Administration (OSHA) and American National Standards Institute (ANSI) standards, along with all applicable codes. List of equipment currently used by WSD - see Exhibit A 3.3. Inspection Contractor Requirements Contractor will: 3.3.1. Provide complete Horizontal Lifeline (HLL) and Vertical Fall Protection System (VFPS) testing and inspection services and certify that the equipment complies with all ANSI standards and is in safe operating condition. 3.3.2. Perform the inspection on-site at the designated WSD facilities, within two (2) working days of department request. 3.3.3. Notify WSD staff when inspection is required on specific equipment and assist in scheduling appointments for inspection. 3.3.4. Provide a detailed inspection and/or repair quote to WSD supervisor or assigned staff, to include all recommendations, fees and charges, prior to commencement of any repairs. 3.3.5. Notify on-site supervisory personnel responsible for the use of the equipment, when equipment cannot be certified without needed repairs. Provide verbal communication regarding the extent of repair required before compliance certification can be made. 3.3.6. Check in with an on-site supervisor at each location, prior to conducting work. Check out with on-site supervisor before leaving each site. 14 Invitation For Bid #IFB-2425-WWC-702 Title: Horizontal Lifeline & Vertical Fall Protection Systems Inspections and Repairs 3.3.7. Red Tag' equipment that has been deemed non-operational. Notify the WSD facility supervisor and/or designated representative within one (1) business day, when the equipment becomes operational and has passed inspection. 3.3.8. Contractor trained and manufacturer certified employees in accordance with industry standards are to conduct Horizontal Lifeline and Vertical Fall Protection System Inspections, and are capable of properly, safely, and promptly providing services requested herein. Non-certified employees are not permitted to perform or assist in any manner. 3.3.9. Provide on-site training to WSD staff on any new equipment purchased. WSD staff to notify Contractor and schedule training(s) as required. 3.3.10. Notify the WSD facility supervisor and/or designated representative when equipment is required to be removed. The contractor will provide WSD with written documentation stating removal of which equipment (clearly identify) and criteria for classifying the removal, or portions of the equipment being removed from City property. 3.3.10.a. The written documentation must include but not limited to: 3.3.10.a.1. Indicate description of item(s) 3.3.10.a.2. Model/serial number(s) of specific equipment removed 3.3.10.a.3. Date when the equipment was removed 3.3.10.a.4. A date when the return of the equipment is expected back at each facility 3.3.11. Conduct non-destructive testing to include, but not limited to: 3.3.11.a. Swage Slip Indicator 3.3.11.b Cable pre-tension 3.3.11.c. Turnbuckle 3.3.11.d. Energy Absorber 3.3.11.c. Intermediate Brackets 3.3.11.d. Line Tensioning Devices 3.3.11.e. Cable 3.4. Repair and Maintenance Contractor Requirements Contractor will: 3.4.1. Perform all work by a skilled tradesman with experience in conducting Horizontal Lifeline (HLL) and Vertical Fall Protection System (VFPS) Inspection and repairs. 15 Invitation For Bid #IFB-2425-WWC-702 Title: Horizontal Lifeline & Vertical Fall Protection Systems Inspections and Repairs 3.4.2. Any repair estimated to exceed $150.00 must be approved by the WSD facility supervisor prior to any commencement of work. Upon completion, Contractor will provide WSD supervisor with written documentation and reports for any technical services, maintenance or repair(s) conducted. 3.4.3. Provide all preventative maintenance and repair services in accordance with the manufacturer's specifications. 3.4.4. Check in with an on-site supervisor at each location, prior to conducting work. Check out with on-site supervisor before leaving each site. 3.4.5. Obtain final approval on all work performed on site. The WSD facility supervisor and/or designated representative is required to sign off on work being conducted prior to leaving the premises. If any repair, maintenance or inspection is unsatisfactory or unacceptable, the WSD facility supervisor will contact the Contractor in writing within three (3) business days from completion of work. The Contractor will have three (3) business days from the date of notification to correct any deficiencies. 3.4.6. Always provide and use fall protection equipment on all job sites. Fall protection includes safety harnesses that are fitted with a lifeline and securely attached to a structural member of the building or properly secured safety nets. 3.4.7. Provide and place barricades, tarps, plastic, flag tape, and other safety/traffic control equipment required to protect its employees, the public, surrounding areas, equipment and vehicles. Directions may be provided by WSD staff on each site. 3.4.8. Manage the flow of vehicular traffic and ensure traffic is not impeded at any time during this project. The safety of the Contractor's employees and the public is of priority to the City and the Contractor must take all necessary steps to ensure proper safety during the performance of the Contractor. 3.4.9. Provide service response within 48 hours of service call. Provide emergency response time of 24 hours from the time of request, unless otherwise scheduled with the WSD facility supervisor. In both instances, the Contractor must provide a call back within one hour to confirm request for service by the City. 3.4.10. Notify the WSD facility supervisor by writing within one working day in advance of any repairs that cannot be serviced. 3.4.11. Track and record all service(s) provided. Records to include but not limited to list by each piece of equipment unique identifier and WSD location. Records to be provided to WSD staff on an as needed basis. WSD may request records without prior notification. 3.4.12. Contractor(s) may be required to provide replacement of equipment the same day as equipment removal. Requirement will be at the discretion and communication of WSD staff. Replacement equipment is to be only with the same model number and manufacturer specifications. 3.4.13. Ensure all replacement parts must meet the specifications of the original manufacturer or approved equal. The Contractor must cover all parts and services with a full 90-day warranty. Any manufacturer's warranty that exceeds the 90-day period will be the responsibility of the Contractor to administer. 16 Invitation For Bid #IFB-2425-WWC-702 Title: Horizontal Lifeline & Vertical Fall Protection Systems Inspections and Repairs 3.4.14. If original equipment manufacturer (OEM) parts are not being used, Contractor to obtain WSD supervisor approval before work is conducted.
