The purpose of this Request for Proposals is to establish contracts for on demand, maintenance and repair services for mechanical, HVAC and plumbing systems in Department of Corrections facilities throughout the state. The contract period will be determined at the time of award, and may be renewed in accordance with RFP section VIII, paragraph D. All proposals received after the appointed date and hour for receipt, will not be considered for an award.Offerors have the sole responsibility for assuring that proposals are received by the designated date and time. The attached Proposal Form (Attachment A) shall be used for submitting proposals. The Proposal Form must be completed and the certification located at the bottom of the form completed and signed by an official who has the authority to commit the Firm. RENEWAL OF CONTRACT: This contract may be renewed by the Commonwealth for a period of four (4) successive one-year periods under the terms and conditions of the original contract except as stated in 1. and 2. below. Price increases may be negotiated only at the time of renewal. Written notice of the Commonwealth's intention to renew shall be given approximately 90 days prior to the expiration date of each contract period.
