OPWDD's Capital District DDSO intends to procure the replacement of a driveway (per the attached scope of work) pursuant to its discretionary purchasing authority. Location- 258 Lapp Rd Clifton Park, NY 12065 To be considered for this project, all quotes must be submitted on the Official Bid Form provided and include the materials and labor cost break down. Vendors / sub-contractors must be registered with NYS DOL in order to submit a quote. A copy of the certificate must be sent to us with your quote (see guidelines in the documents tab.) Service-Disabled Veteran-Owned Set Aside: No Contract term: 120 Days of Award Replace driveway according to designs This project is regarding the driveway enlargement. The vendor shall provide all labor and materials necessary to complete the work in a professional manner. Work is to be completed within 45 days of award, unless modified below. This is a NYS Public Works project, and prevailing wages are to be paid. Refer to attached designs for the driveway specifications. Site must be cleaned, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be secured. Upon completion of the driveway, before removal of equipment, the contractor is to schedule a quality control and quality assurance inspection with the Maintenance Supervisor. SCOPE OF WORK includes, but is not necessarily limited to the following: o Refer to the attached designs for all related specifications. o These designs are to be followed unless stated otherwise. o Any deviations from the designs must be approve through Capital District DDSO first. o Any unforeseen concerns found during this project, Capital District DDSO MUST be notified before work continues. o All waist material must be removed from the property. o The Contractor will follow all current NYS building and safety codes. o The house will be occupied during the project, vendor will allow the normal operations of the day to continue. o Access to the stairs and front ramp must be kept clear for wheelchair access.
