Project: Lloyds Chapel Road (405 Lloyds Chapel Road Church Hill, TN 37642) Scope:- The contractor selected will be responsible for repairing defective area along the roadway. Scope of Work For scope of work, the bid should include traffic control, associated vegetation matter removal, clearing, cleaning, grading, and any necessary saw cutting, subgrade preparation, base installation. Bidders shall place their bid in a sealed envelope along with a bid bond and certificates of insurance. The outside of the envelope must indicate the bidder's name, license number, expiration date, and that part of classification applying to the bid in accordance with TCA 62-6-119. Bids not conforming to these provisions shall not be opened. Each bidder agrees by the submission of his bid to commence work within fifteen (15) days of the issuance by the City of a "Written Notice to Proceed" and to fully complete the work within forty-five (45) calendar days from the date of the Notice to Proceed. The successful bidder will be required to furnish a Performance and Payment Bond. Installation shall meet TDOT Standard Specifications for Road and Bridge Construction. No bidder may withdraw his bid within thirty (30) days after the date of the opening thereof. The work shall have a one-year warranty secured by a bond. The City of Church Hill reserves the right to reject any and/or all proposals and to award the bid in any manner deemed to be in the best interest of the City. 405 Loyds Chapel Road- Uprooted Tree at Roadway The work described consists of furnishing all traffic control, associated vegetation matter removal, clearing, cleaning, prime coat, tack coat, and any necessary incidental work required to install an asphalt overlay on the street. Remove the damaged tree and any related debris. 1. Design a retaining wall using precast concrete blocks and caps. a. The wall should extend at least one full block height above the finished grade of the road. b. The wall should be no longer than necessary to support the exposed edge of the road. c. The design of the wall will be approved by the Mayor prior to construction. d. The wall should be as close to the road as possible. e. Install retroreflective panels on both ends of the wall for oncoming traffic awareness. 2. Fill the area between the existing road and the wall with compacted 57 stone. 3. Repair the damage to the existing road by excavating the damaged areas. 4. Slope the edges of the detention pond to allow for better clearance for the homeowner. 5. Repair any damage to the existing gravel driveway and seed and straw over any damaged grass.
