Pd 25-024 Specifications for The Repair, Replacement of Existing and Installation of New Overhead Doors for the County of Gloucester and Existing Units Within the County as Allowed Through the County Contract System Numbers Ck-01-gc & 16glcp Bids will be made on the proposal form provided in the manner designated therein and required by the specifications enclosed in a sealed envelope and addressed to the Purchasing Agent, 2 South Broad St, Woodbury, New Jersey 08096, and clearly marked on the outside "date, time and name of the item being bid". Surety in the form of a bid bond, certified check or cashier's check in the amount of 10% of the bid, but not to exceed $20,000.00 made payable to the treasurer will be required with this bid. A performance bond in the amount of 100% will be required of the successful bidder. The bid must be accompanied by a Non-Collusion Affidavit. Public Disclosure, N.J.S.A. 52:25 24.2 provides that no corporation or partnership shall be awarded any State, County, Municipal or School District contract for the performance of any work or the furnishing of any materials or supplies, unless, prior to the receipt of the bid or accompanying the bid of said corporation or partnership there is submitted a statement. The statement shall set forth the names and addresses of all stockholders in the corporation or partnership who own 10% or more of its stock or any class, or of all individual partners in the partnership who own a 10% or greater interest. FAILURE TO SUPPLY THIS INFORMATION WITH YOUR FORM OF PROPOSAL WILL BE CAUSE TO REJECT THE BID.
