Contractor shall provide all necessary materials, supplies, tools, equipment, transportation, licenses, permits, prevailing wages, insurance, bonds, and personnel to: provide the DSH-Patton with roof covering removal and reinstallation of new roof covering and other work as per architectural plans and specification. 2. The DSH-Patton is a forensic psychiatric hospital located in Patton, CA, in San Bernardino County. DSH-Patton was established in 1890 and opened in 1893. DSH Patton provides treatment to forensically and civilly committed patients within a secure treatment area. The hospital does not accept voluntary admissions. 3. The DSH-Patton is not permitting the use of subcontractors for this service. 4. Please carefully review and consider the minimum qualifications, and the detailed Scope of Work located in Attachment 18, Sample Agreement, Exhibit A, Scope of Work, in order to complete your bid. B. Agreement Term and Security Provisions: 1. The term of any agreement resulting from this IFB is anticipated to be July 1, 2025, through June 30, 2027. Any agreement resulting from this IFB shall have no force or effect until it is signed by both parties and approved by the Department of General Services (DGS), if such approval is required. Performance shall start no later than on the expressed date set by the awarding agency and after all approvals have been obtained and the agreement is fully executed. 2. All performance, under the agreement, shall be completed on or before the termination date of the agreement. 3. Should the Contractor fail to commence work at the agreed upon time, the DSH reserves the right to terminate the agreement upon five (5) calendar days written notice to the Contractor. 4. Contractor shall: Remove approximately 12,360 sq. ft. of existing shingles, 2,454 square feet of roofing on low slope, underlayment, roof edging, counter flashing, nails, etc. Contractor shall repair any damage to substrate. Remove and replace all existing fascia with new 2 x 8 (nominal) kiln-dried spruce fascia. Remove and replace all existing soffit vents. Install new roof vents as per plans and specifications. Install new roofing system as per specifications and architectural plans. Roofing installers to be GAF Master Elite Roofing Contractor. All work shall be governed by the architectural plans and specifications as approved by the Office of the State Fire Marshal (OSFM). (Contractor shall be responsible for all final measurements).
