For furnishing all labor, materials and equipment and performing all work necessary to: Base Bid No. 1: Construct T-Hangar Taxilane Base Bid No. 2: Rehabilitate Main Taxilane and Apron Base Instructions for using Bid Express, Project Bids, and the electronic bidding process are available from the Office Engineer Division. Any firm bidding a project as a joint venture, that has not been previously approved by the department, must complete the 'Request for Joint Venture' form, which is posted on the ODOT website, prior to bidding. Unless otherwise specified in the proposal, all bids must be submitted with a proposal guaranty equal to 5% of the bid total. Written bids must have their bond stapled to the back of the proposal. In lieu of a bid bond, a certified/cashier's check made payable to the Oklahoma Department of Transportation may be submitted as a proposal guaranty. Checks must be delivered to Office Engineer Division prior to the bid opening. Proposal checks will be held or returned by the Department as per Subsection 103.04 of the State Standard Specifications. The Contractor is required to comply with wage and labor provisions and to pay minimum wages in accordance with the schedule of wage rates established by the United States Department of Labor. This project is subject to the requirements of 49 CFR Part 26 Disadvantaged Business Enterprise Participation. The owner has established a contract goal of 6.50 percent participation for small business concerns owned and controlled by qualified disadvantaged business enterprises (DBE). The bidder shall make and document good faith efforts, as defined in Appendix A of 49 CFR Part 26, to meet the established goal. Award of contract is also subject to the following Federal provisions: Executive Order 11246 and DOL Regulation 41 CFR PART 60 - Affirmative Action to Ensure Equal Employment Opportunity DOL Regulation 29 CFR Part 5 - Davis Bacon Act DOT Regulation 49 CFR PART 29 - Governmentwide Debarment and Suspension and Governmentwide Requirements for Drug-free Workplace DOT Regulation 49 CFR PART 30 - Denial of Public Works Contracts to Suppliers of Goods and Services of Countries that Deny Contracts to Suppliers of Goods and Services of Countries that Deny Procurement Market Access to U.S. Contractors (Foreign Trade Restriction). This item consists of furnishing all labor, materials and equipment for temporary construction items necessary for the safe and proper execution of work not otherwise included in other contract bid items. The Contractor will be expected to procure, install, utilize, operate, relocate as necessary, maintain, repair and replace, and remove the temporary construction items listed below and other items contained in the plans and specifications. Temporary construction items to be provided may include, but are not limited to: flagmen; gate guards; escorts; radios; portable lighting units; steel plates for temporary covering of excavations and structures; barricades; delineators; temporary haul roads; temporary signs; test pits; vacuum trucks and sweepers; wheel wash stations; portable sanitary facilities; temporary construction staging areas; waste disposal containers; taxi closure markers; temporary edge light coverings, centerline light masking, and sign coverings / modifications; temporary electrical cables, connections, and jumpering; and electrical lockout and tagout. a. General description. The Contractor shall establish a CQCP to perform QC inspection and testing of all items of work required by the technical specifications, including those performed by subcontractors. The CQCP shall ensure conformance to applicable specifications and plans with respect to materials, off-site fabrication, workmanship, construction, finish, and functional performance. The CQCP shall be effective for control of all construction work performed under this Contract and shall specifically include surveillance and tests required by the technical specifications, in addition to other requirements of this section and any other activities deemed necessary by the Contractor to establish an effective level of QC. b. Contractor Quality Control Program (CQCP). The Contractor shall describe the CQCP in a written document that shall be reviewed and approved by the RPR prior to the start of any production, construction, or off-site fabrication. The written CQCP shall be submitted to the RPR for review and approval at least 10 calendar days before the CQCP Workshop. The Contractor's CQCP and QC testing laboratory must be approved in writing by the RPR prior to the Notice to Proceed (NTP). The CQCP shall be organized to address, as a minimum, the following: 1. QC organization and resumes of key staff 2. Project progress schedule 3. Submittals schedule 4. Inspection requirements 5. QC testing plan 6. Documentation of QC activities and distribution of QC reports 7. Requirements for corrective action when QC and/or QA acceptance criteria are not met 8. Material quality and construction means and methods. Address all elements applicable to the project that affect the quality of the pavement structure including subgrade, subbase, base, and surface course. Some elements that must be addressed include, but is not limited to mix design, aggregate grading, stockpile management, mixing and transporting, placing and finishing, quality control testing and inspection, smoothness, laydown plan, equipment, and temperature management plan. The Contractor must add any additional elements to the CQCP that is necessary to adequately control all production and/or construction processes required by this contract. This item shall consist of underground electrical conduits and duct banks (single or multiple conduits encased in concrete or buried in sand) installed per this specification at the locations and per the dimensions, designs, and details shown on the plans. This item shall include furnishing and installing of all underground electrical duct banks and individual and multiple underground conduits and removal of existing duct banks. It shall also include all turfing trenching, backfilling, removal, and restoration of any paved or turfed areas; concrete encasement, mandrelling, pulling lines, duct markers, plugging of conduits, and the testing of the installation as a completed system ready for installation of cables per the plans and specifications. This item shall also include furnishing and installing conduits and all incidentals for providing positive drainage of the system. Verification of existing ducts is incidental to the pay items provided in this specification. Contractor is to complete the Project in accordance with the Project Documents and in accordance with all codes and regulations governing the construction of the Project. Any work, materials, or equipment that may be reasonably inferred from the Project Documents as being required to produce the intended result shall be supplied by Contractor whether or not specifically called for. Reference to standard specifications, manuals, or codes of any technical society, organization or association, or to the laws or regulations of any governmental authority, whether such reference be specific or by implication, shall mean the latest standard, specification, manual, code, or laws or regulations in effect at the time of opening of bids and Contractor shall comply therewith. Sponsor shall have no duties other than those duties and obligations expressly set forth within the Contract Documents.
