Statements of qualifications of more than five pages will not be accepted. Page 1 - Firm Information (15 pts) Name, address, and brief description of firm. Number of years in business. Identify the specific office that would be working on the project if selected. Include a listing of previous experience with the Village of Silver Lake. Page 2 - Project Team Qualifications (20 pts) Include the project manager and key team members resumes. Page 3 - Client References (15 pts) Include contact information (community, name, phone, address, email) for three current projects which the project manager and team is working on. Page 4 - Past Experience (20 pts) Include three examples, one paragraph each, of similar projects with the proposed project manager and team. Page 5 - Project Specific Qualifications (30 pts) Include a one-page narrative describing the firm's interest in the project, particular abilities and qualifications related to this project, experience administering federal grants, and reasons why the firm should be selected as the consulting firm. The Village of Silver Lake seeks to hire a professional consulting firm to provide due diligence, design, funding solicitation, construction engineering, and potential grant administration services associated with the reconstruction of water, sewer, stormwater, and roadway infrastructure within the public right-of-way. General limits stretch along Silver Lake Boulevard from Kent Road to Bellaire Road. The project will address aging infrastructure, reoccurring failures, and may be phased based on successful funding acquisition.
