Prequalification Requirement: All contractors wishing to submit a bid must first complete and submit the County's Prequalification Form (Attachment C) no later than ten (10) calendar days prior to the bid deadline. The County will review prequalification submissions and notify contractors whether they are approved to bid. Prequalification requirements are as follows: Submission of references and list of similar demolition/abatement projects for government agencies completed within the past twelve (12) months Demonstrated history of satisfactory past performance and reasonable change order management Proof of compliance with DEQ regulations (no outstanding violations; disclosure of any fines within the past five (5) years) This project is subject to Oregon's prevailing wage rates. All Bids must be accompanied by a certified or cashier's check, irrevocable letter of credit (Bank), or Bid Bond, payable to the Klamath County in an amount equal to ten percent (10%) of the total amount of the Bid. Responses to questions submitted by the deadline will be compiled and issued as an RFI Addendum.
