Reference Number cb-990-83316102 The contractor must provide a maintenance Log for each elevating device, in a manner that will identify the Contractor's adherence to the Maintenance Services, and the applicable Codes and Standards, as described in the Safety Codes. The Project Authority will use this Log as proof of service delivery should there be a discrepancy between services the contractor has rendered, and the services invoiced. The contractor must adhere to the maintenance requirements and intervals provided within the Elevating Devices Log as a minimum. Should the contractor deem that the maintenance requirements and intervals need to be increased for the elevating devices, the contractor must advise the Project Authority who must approve the increased services. The contractor must note the increased maintenance requirements and intervals in the Maintenance Logbook. The contractor must register all work performed on the equipment under this contract in maintenance logbook must include the equipment number, date, time of arrival, contracts employees s name, nature of the work performed, any identified problem(s) and actions taken to rectify them and time of departure. The Contractor must provide the Logbook at the start of the contract, which must be kept in the elevator machine room. The Project Authority will provide any site-specific examination and test frequencies for this contract which is provided in the tasks on statement of work. When the contractor cannot return malfunctioning elevating equipment to service within the same day, the Contractor must provide, by the end of the following working day, a written report to the project authority describing the nature of the problem and the expected date of the service resumption. When the contractor determines it is necessary to take all or part of the elevator system(s) out of service for inspections, tests maintenance services and repairs, the contractor must make arrangements with the project authority a minimum of seventy-two (72) hours in advance). The contractor must provide details of the devices to be taken out of service electronically to the Project Authority outlining the scope of the work to be performed, anticipated time frame and the equipment involved. The Contractor must use proven information collection and delivery techniques, methodologies and systems as necessary to efficiently meet CSC requirements for regulatory reporting and documentation as well as invoice verification. The contractor must ensure that computer systems and information are protected with due regard to security and ensure disaster recovery and backup plans and procedures are in place to protect that information. The contractor must provide copies of all maintenance related work, tickets and visitation records with the invoices to allow the Project Authority to verify that the contractor meets inspection and maintenance frequencies stated in the contract.
