Water Main Repairs, Water Service Maintenance, Sewers - Rehabilitation, Quotes, Installation of Sewer Pipelines, Televising Sewers, Supplies, Materials handling, Mechanical, Storm water Improvements, Piping, Material Testing, Goods & Services, Structural, Miscellaneous, General Contractors - Construction The work included in this Contract shall consist of furnishing all the materials, labor, equipment and incidentals required for cured in place pipe (CIPP) water main lining at five (5) separate locations, valve vault installations, butterfly and gate valve replacements, including patching, landscape restoration, tree protection, combination concrete curb and gutter removal and replacement, and all other incidental work necessary to complete this improvement according to the Contract Documents. 2. Work Site: The work shall be performed at the locations specified below: 1543 Berkeley Road - _ 241 LF 8" CIPP. 1350 Old Skokie Valley Road - _ 538 LF 6" CIPP. 1300 Old Skokie Valley Road - _ 348 LF 6" CIPP. 1252 Old Skokie Valley Road - _ 296 LF 6" CIPP. 1200 Old Skokie Valley Road - _ 239 LF 8" CIPP. 1207 Park Ave W - 1 EA Valve Vault, 5' Dia., with 14" Butterfly Valve 1620 Edgewood Road - 1 EA 8" Valve Replacement & 1 EA Valve Vault, 5' Dia., with 12" Valve The work included in this Contract shall consist of furnishing all the materials, labor, equipment and incidentals required for cured in place pipe (CIPP) water main lining at five (5) separate locations, valve vault installations, butterfly and gate valve replacements, including patching, landscape restoration, tree protection, combination concrete curb and gutter removal and replacement, and all other incidental work necessary to complete this improvement according to the Contract Documents. 2. PROJECT LOCATION The work shall be performed at the locations specified below: 1543 Berkeley Road - _ 241 LF 8" CIPP. 1350 Old Skokie Valley Road - _ 538 LF 6" CIPP. 1300 Old Skokie Valley Road - _ 348 LF 6" CIPP. 1252 Old Skokie Valley Road - _ 296 LF 6" CIPP. 1200 Old Skokie Valley Road - _ 239 LF 8" CIPP. 1207 Park Ave W - 1 EA Valve Vault, 5' Dia., with 14" Butterfly Valve 1620 Edgewood Road - 1 EA 8" Valve Replacement & 1 EA Valve Vault, 5' Dia., with 12" Valve 3. PRECONSTUCTION VIDEO RECORDING (HP-002) Description: This work shall be done in accordance with the Standard Specifications insofar as applicable. Materials: This work shall include providing the City with detailed recording of the construction site. Construction Requirements: Detailed recording of the construction site, in DVD format, shall be taken and submitted to the City Engineer for approval prior to project mobilization and start of construction. Included in the item is complete documentation of project site existing conditions, location of all objects which include street signage, road, driveways, trees, landscaping and mailboxes along the project route. The video fill shall be submitted in a format that can be played by conventional video players such as Microsoft Video Editor. The media used may be a USB flash drive, a portable HDD or SSD, or a DVD compact disc. Media submitted shall become the property of the City and shall not be returned to the Contractor. Method of Measurement: This work shall be measured for at the contract unit price per lump sum. Basis of Payment: This work shall be paid for at the contract unit price per lump sum, for PRE CONSTRUCTION VIDEO RECORDING which price shall include all labor, equipment and materials required to complete the work. 4. PROGRESS MEETINGS (HP-003) Weekly progress meetings will be held with the City and the Contractor. The City will send written notification to the Contractor informing him of the date, time and location of the progress meeting. The Contractor's superintendent or staff member responsible for the work shall be present at the meeting. At the City's discretion the meetings may be held on-site. If the meetings are not held on-site, the City will provide information to hold a virtual meeting via Zoom. At each meeting the Contractor shall provide the City with a construction schedule. The construction schedule will show future work planned for a two (2) week period out from the date of the progress meeting. Also, the contractor will provide the City with an update on work completed the week prior to the progress meeting. 5. CONSTRUCTION SEQUENCING AND FAILURE TO COMPLY (HP-004) After the completion of underground work, the Contractor is required to begin pavement work within two weeks. At the Weekly Progress Meeting, the Contractor will be informed of this schedule. Failure to adhere to these strict timelines will result in the assessment of fines to the Contractor in the amount of $500.00 per calendar day. After placement of binder course, the Contractor is required to begin placement of final asphalt surface within two weeks. At the Weekly Progress Meeting, the Contractor will be informed of this schedule. Failure to adhere to these strict timelines will result in the assessment of fines to the Contractor in the amount of $500.00 per calendar day.
