Reference Number 0000299011 Question Acceptance Deadline 2025/08/08 11:59:00 PM EDT Pricing Lump sum Hydrofluorosilicic Acid (H2SiF6) also known as "HFS" is required for the use in the treatment of the City of Ottawa's water supplies. The HFS material supplied must: (a) Be a nominal twenty five percent (25%) percent solution and conform to American Water Works Association (AWWA) Standard B703-11 (or most recent revision) for Fluorosilicic Acid; and (b) The HFS must be certified as suitable for contact with or for use in the treatment of drinking water by an accredited certification organization, in accordance with ANSI/NSF Standard 60, Drinking Water Treatment Chemicals- Health Effects. This accreditation must be held and maintained throughout the entire period of contract. It is the responsibility of the Contractor to inform the City within twenty-four (24) hours in the event that ANSI/NSF certification has been revoked or lapsed. Commercial General Liability insurance issued on an occurrence basis for an amount of not less than $5,000,000 per occurrence for any negligent acts or omissions by the Contractor. Such insurance shall include, but is not limited to, bodily injury, death and property damage including loss of use: premises, property and operations liability; products and broad form completed operations liability; blanket contractual liability; cross liability; severability of interest clause; contingent employers liability; personal injury liability; owner's and contractor's protective coverage; non-owned automobile liability; broad form property damage; employees as Additional Insured; and occurrence property damage. The Commercial General Liability insurance policies shall be in the name of the Contractor and shall name the City of Ottawa as an Additional Insured.
