Renovation of a municipal facility in Rogers, Arkansas. Completed plans call for the renovation of a municipal facility.

The City reserves the right to reject any and all bids. Work will include the following trades: PEMB Building Component Supply & Erection, Roofing, HVAC, Plumbing, Electrical, Electronic Safety and Security. Bids must be submitted on the supplied bid form. The bid form for each trade package can be found in Milestone Construction's Bid Manual. Bidders must acknowledge all addenda and Construction Manager's Clarifications on the bid proposal form. Successful bidders providing a complete, responsive, and responsible bid will be required to execute a Master Service Agreement and Project Specific Contract with Milestone Construction Company. Bidders must be prepared to provide insurance as specified in the Construction Manager's Bid Manual portion of the bidding documents. See the Proposal Procedures Section of Milestone's Bid Manual for all special requirements for bidders and bidding. Please note that successful bidders will be required to sign Milestone Construction Company's standard MSA, subcontract / purchase order with no modifications and follow Milestone's safety policies. Copies of these documents are available for review in Milestone's Bid Manual. Subcontractors will be required to furnish insurance as outlined in the specifications, as well as provide a waiver of subrogation and list Milestone Construction, the City of Rogers, and Paragon Architects as an additional insured. For any questions regarding our insurance requirements, please contact our office. The Owner reserves the right to accept any proposal, to reject any or all proposals, or to waive any information in proposals received where such acceptance, rejection, or waiver is considered to be in the best interest of the Owner. The Owner reserves the right to waive any informalities or irregularities in procedure. No proposal may be withdrawn for a period of sixty (60) days subsequent to date of the opening of proposals without consent of the owner. The Owner hereby notifies all submitters that this contract may be subject to applicable labor laws, non-discrimination provisions, wage rate laws and other or federal laws, including the Fair Labor Standards Act of 1938. The Work Hours Act of 1962 and Title VI of the Civil Rights Act of 1964 may apply. The City of Rogers complies with all civil rights provisions and federal statutes and related authorities that prohibit discrimination in programs and activities receiving federal financial assistance. The City of Rogers encourages all qualified small, minority and women business enterprises to bid on and receive contracts for goods, services, and construction. Also, City of Rogers encourages all general contractors to subcontract portions of their contract to qualified small, minority and women business enterprises. The Project consists of the repair of roofing, exterior walls, light fixtures, electrical devices, and mechanical equipment damage. General Requirements: 1) Full-time, on-site personnel dedicated to supervision for duration of time that subcontractor is on site. Supervision to include coordination with other trades, layout of all this subcontractor's work, all necessary reporting for the project including subcontractor daily reports, safety meetings, checklists, inspections, anddelivery coordination to meet project schedule. As-Built Drawings to be updated daily and as required. 2) This subcontractor will be responsible for field measurements and layout as required for this Contractor's work. Milestone will provide a benchmark/reference point to begin layout. 3) Unless otherwise noted, payment for third-party testing is by others. Milestone's Superintendent is to schedule all testing but this Subcontractor will be responsible for coordination with Milestone's Superintendent and the testing company for scheduling time and accommodations as required to allowing the testing to be performed. This Subcontractor will be responsible for payment for re-testing of any testing failed due to the actions of this Subcontractor or this Subcontractor's vendors. 4) Haul off of all spoils as a result of this subcontractor's work as required. 5) Multiple mobilizations if required. 6) All fees, permits, certifications, etc. as required for this scope of work. This will include any applicable sales, use, remodel or other taxes required in the jurisdiction of this project. 7) Refer to Attachment "A" for insurance requirements for this project. 8) Any required submittals, samples, mockups or sample panels, shop drawings, etc. for this scope of work are to be provided within two (2) weeks of receipt of this contract unless noted otherwise. 9) Coordination with other trades. 10) Continuous clean up of trash and removal of debris into dumpster or trash receptacles. 11) Continuous clean up of mud, rocks, dirt, etc. from personnel, trucks, and equipment onto roads, sidewalks, and surrounding areas as required. 12) All scaffolding, equipment, lifts, hoisting, material and tools required to complete this scope of work.
