Remodeling and renovation of a residential development in Milwaukee, Wisconsin. Completed plans call for the remodel of a residential development; and for the renovation of a residential development.
Scope of work Gladys mosher, 1739-1739a s. 6tb st, (414)232-9620 Rehab specialist: elona peace (414) 708-5362 Loan officer: judy carson(414) 286-3543 First inspection date: 6/26/25 prior version dates: Both the "Rehabilitation and technical specifications and performance standard for the city of milwaukee february, 2006" and the "Nidc addendum to specifications," dated 8/23/07 & 5/13/08, are incorporated into this scope by reference. These items provide an outline of materials requirements & performance expectations. Updated copies can be obtained from nidc @ 809 n broadway-3rd flr. Lead related work: any task that a disturbs previously painted surface shall be performed by properly certifiedpersonnel and follow the state of wisconsin department of health services administrative code chapter dhs 163 regarding the certification for the identification, removal and reduction of lead-based paint hazards (pb). And, if applicable: oyes no Any task that disturbs a previously painted surface requires milwaukee health department notification (pb-n). Window replacement shall be performed by properly certified personnel and requires a milwaukee health department permit (pb-a). Miscellaneous: the contractor is responsible to field verify all measurements, the amount of materials needed, and the number of windows in the building. If any item in this scope calls for a certain manufacturer, model number, or approved equivalent of a particular item, and that item is to be substituted, both nidc and the owner must approve the substitution in writing as a part of the contract. 1 site 2 Trim all tree branches encroaching on house walls and porch (s/e) elevations, Minimum 8' clearance Remove and replace concrete service walk, south negatively pitched, pooling Toward house/porch foundation wall form, grade, pour and broom finished all New concrete. Include all necessary reinforcements, expansion felt etc. Backfill 3 with screen topsoil, seed and straw. 4 5 porch(es) 6install new heavy duty aluminum or iron guard/step railing both open sides to, Front concrete porches of both units, ensure mechanically fasten, patch concrete Step 1739 (lower unit), as needed, to code 7 remove and replace southeast (2nd floor porch) decking, and guard railings. Install epdm rubber membrane to manufactures specifications, and install new Metal guard railings, to code 8 repair front porch eaves (fascia & soffit) remove and replace rotten wood trim. Install aluminum wrap (cladding) to match the existing. Prepare north lower Peeling window seal to match Pr Pr Pr-pb (permit) Pr-pb (permit) Pr-pb 9 10 exterior house 11 provide bricks and set/point missing bricks. Match brick color, (do not used any Cream city brick to make repairs) opening south (int/ext.), some wood framing Maybe required. Tuck-point and parge south brick foundation wall, as needed in Assorted areas, include north wall (minor), use n-type mortar to make above Ground repairs pr 12 remove and replace defective house 5" gutters and ds, front (southeast), rear (southwest) gutters with 5" new k-style gutters and replace missing or defective section of ds, route the same as existing. Repair existing north (alley side) kstyle gutters, secure, and install additional straps, elbows (a) attached to drop tube and defective ds (crush section). Adjust if needed, ensure all gutter are pitched for proper run-off. Install gagas adapters to all ds receivers or comparable. Ensure to seal all gutter junctions, as need with approved gutter Sealant, as needed pr 13 remove and replace southwest, side entry security door deadbolt lockset (single Cylinder), turn knob. Install security door closure to front (south), 1739 (lower Unit) porch, ensure doors are operable pr-pb 14 prepare and paint north lower peeling window trim/sill, to match exsiting color pr-pb 15 16 interior 17 installsurface mounted race way condiut from wall switch to ceiling fixture front East lower unit (1739) bedroom. Replace defective 1st floor unit (north bedroom) Ceiling fixture and the closet wallfixture. 18 replace flex drop tube with rigid tub to 1st floor bathroom drain 19 install missing trim at perimeter of 2nd floor rear entry unit door, owner Responsible for painting Pr-pb (permit) Pr Pr-pb 20 install missing electric outlet (receptacles) and switch cover plates in 2nd floor Unit (1739a) pr self-help 21 install new combination 10 year sealed battery smoke and carbon monoxide Detectors in basement, 1st floor and 2nd floor. Through-out entire dwelling, to pr self-help 22 23 basement 24 install missing electric ceiling junction cover plates pr 25 26 total scope estimate 27 28 29 prepared by: date: 30 31 32 owner approval: date: Start and completion of work The work presented in this project is to be started within 15 days after contractor's receipt of a notice to proceed and is to be satisfactorily completed within 45 days thereafter. Interior/exterior completion and/or weather-affected repairs, if any, must be completed as further outlined and incorporated in bid attachments a and/or b respectively. (strike if not applicable.) Small business enterprises (sbe) and section 3 work force participation contractors are notified that it is nidc policy to strongly encourage sbe and section 3 participation on all Nidc-sponsored contracts. Information about the city of milwaukee's sbe programs is available at online. Bids may be withdrawn only in total, and only by a written request prior to the award of the contract. Owner has sole discretion to grant a request for a bid withdrawal, and only in the case of an error that will result in a significant financial hardship. The owner reserves the right to reject any and all bids and to waive informalities.
